A review of three staff records indicated the following: two did not have documentation of observations done at least on a bi-monthly basis and one did not have evidence of a current EEC PQ registration.
A review of two IHCPs indicated that designated staff were not identified to administer the medication in case of an emergency,

The following was observed and accessible to children: On the INFANT/TODDLER outdoor space, the ground covering was curling up creating a potential trip hazard. On the PRESCHOOL outdoor space, there were mushrooms on the side of a wooden log, there were weeds growing along the fence on one side, and there was chipped paint on the blue picnic table.
A plan for diapering and toileting was not posted in the first children's bathroom.
A review of medications indicated that one was not stored with its original packaging with prescription label attached and one over-the-counter medication expired April 2024.
This investigation inspection recorded no violations or advisories.

It was observed that the play kitchens in the Toddler and Preschool classrooms were not secured and tippable.
It was observed in the Infant classrooms that two children's sippy cups were not labeled with the individual child's name for whom they are intended.

A review of three First Aid Kits indicate the following: Three were missing a cold pack, two were missing a CPR mouth guard and two were missing scissors.

It was observed that in the kitchen, in two unlocked cabinets, there were two bottles of cleaning solution and a bottle of sanitizer tablets with manufacturer warnings to “keep out of reach of children”.

It was observed that a current staff schedule was not posted in an area easily visible to educators, parents and visitors.
It was observed that the wooden border towards the back right side of the playground was cracked and splintered.
It was observed that twp children's emergency medications had expired in July 2023.
It was observed that in the first bathroom, there was an open box of gloves and wipe packages accessible to children. These are a potential choking hazard.
A review of three children's records indicated the following: -Two were not updated annually since their enrollment date -All three did not have evidence of a current parental medication consent form for at least one medication.
It was observed that in the Toddler classroom next to the changing table, a broom and dustpan were accessible to children.
It was observed and accessible to children that there was a metal rake and a piece of metal grate to the side of the Preschool playground entrance.

The following were observed outdoors: -there was a tippable play kitchen on the Infant/Toddler playground. -there were protruding bolts on the outside of the preschool playground gate.
The following were observed: -In the staff bathroom where the preschool children wash their hands, an aerosol spray was accessible to children. -In the toddler classroom, hand sanitizer located in the unlocked red cabinet was accessible to children. -In the same red cabinet, a personal purse was accessible to children.
