A review of three staff records showed that one educator had not completed any training hours.
A review of 10 children’s records showed the following: one child’s lead results were missing, two children were missing a physical, and two children did not have signed medication consent forms. The licensor advised the educator to review all children’s records and ensure that all documentation is up to date.
During the visit for smoke detector testing, the educator was unable to test the smoke detector in the home.
A review of the first aid kit showed that 11 emergency cards were expired.

During the visit, it was observed that two children with documented allergies were missing signed medication consent forms.

During the visit, it was observed that one exit in the house did not have an exit sign posted.
During the visit, the educator stated that she has plug-in carbon monoxide detectors at both the School House location and her licensed home child care space. However, she was unable to test the detectors at the time of the visit because she is unsure how to operate them. This is a safety hazard, as regulations require that carbon monoxide detectors be properly installed and functioning to ensure the safety of children in care.
During the visit, it was observed that one kitchen play set was not secured to the wall, and boxes of wipes and gloves were accessible to children.

The program provides all meals and snacks, however the menu available did not include breakfast or snacks provided each day.
The provider did not document when the alarm system and alarms were tested as required.
There were some sharp items, (tape dispensers, tools, misc small items) within reach of children in the kitchen drawers. There were some small bottles of alcohol within reach on the kitchen counters. The bird cage and bird were accessible in the dining room.

Written progress reports were not provided to parents as required. Every 3 months for infants and 6 months for toddlers and preschoolers.
Two of the children's records reviewed had not been updated annually.
There were disinfecting wipes within reach of children in the play room. There was a bin shelf that was not secured in the playroom.
There was one child with a documented (REDACTED) The (REDACTED) was not posted. Discussed that the allergy list must be posted prominently and confidentially.
Records were reviewed for all 14 enrolled children; 2 of the children were missing the first few pages of the enrollment packet; 2 children were missing signatures of the Acknowledgment of the Parent Handbook; 3 children were missing current medical documentation; 2 children were missing Annual Update forms.
There were 11 children with the Educator and the Certified Assistant when Licensor arrived.

There was one child who was present at the program when Licensor arrived, and was not documented as present on the attendance records. The Educator stated that the child attends the program on other days, and was unaware that she would be coming by with the Educator’s mother (who is also a certified assistant). Times of arrival and departure were documented for all other children.
Records were reviewed for the two assistants. One of the assistants did not have 30 professional development training hours; she had 12. Discussed that the Educator and the assistants need 10 hours of professional development training annually.
The first aid kit was missing scissors and did not have Emergency Cards for all enrolled children.
The refrigerator temperature exceeded 40 degrees. Discussed that the temperature must be between 32 and 40 degrees.
There were 11 children present when Licensor arrived. Six of the 11 were under the age of 2 years old. Four of the children were infants.
