The following was observed outdoors on the INFANT/TODDLER area: 1) There were two plastic basketball hoops unsecured and tippable. 2) The plastic play kitchen against the play house was unsecured and tippable.
The following was observed: 1) In the INFANT/TODDLER classroom, the wooden play kitchen was unsecured and tippable. In the TODDLER/PRESCHOOL classroom, the play kitchen was unsecured and tippable and the soft mat in the quiet cube had multiple tears.

It was observed in the PREK classroom that there was a loose electrical cord unsecured accessible to children.
It was observed in the INFANT/TODDLER classroom that there were two milk bottles and a water cup that were not labeled with the child's name for whom they were intended.

It was observed in the INFANT/TODDLER classroom that the refrigerator temperature read above 40 degrees.
It was observed in the PREK classroom that there was chipped paint in the Dramatic Play area.
It was observed that a thermometer was not located in the TODDLER/PRESCHOOL classroom refrigerator.
Chipped paint was observed in the Preschool and PreK classrooms.
A review of four children's records indicated the following: three did not have evidence of an IEP despite receiving support services offsite and one did not have evidence of a current custody order since the previous expired in (REDACTED).
A review of four staff records indicated the following: three did not have evidence of an interview, four did not have a completed program orientation form, and two did not have inconsistent evidence of bi-monthly observations.
Staff meeting documentation
A review of the staff meeting log for 2023-2024 indicated that there were five that did not include the duration time and there was no evidence of a meeting conducted in September 2023, January 2024, and May 2024.

A review of the evacuation drill log indicated that a drill was not conducted or documented during March and April 2023.
The following was observed and accessible to children: -In the INFANT/TODDLER classroom, the refrigerator had a crack in the bottom corner of the door. -In the TODDLER/PRESCHOOL classroom, one of the handles on the changing table cabinet had a missing screw and was loose.

Three children with an identified special need did not have progress reports conducted in three month increments.
A review of the injury and incident logs indicated that the Infant/Toddler log did not have entries after March 2023 despite having copies of reports filed and the Preschool log did not have complete information for some entries and said, “see report.”
It was observed that on the INFANT/TODDLER outdoor play area, the plastic tool bench was unsecured and tippable and there was an exposed sharp edge on the base of the red play school.
It was observed that in the TODDLER/PRESCHOOL classroom, there was rust on the air vent that was accessible to children.
It was observed that on the PRESCHOOL outdoor play area, there were splintered and cracked logs. Both outdoor play areas had unknown vegetation growing along and through the fencing.
It was observed that some children's cups were labeled with the child's name throughout the program. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes, and sleeping materials, must be labeled with the name of the child for whom they are intended.
A review of two children's files indicated that one did not have evidence of an IHCP for a diagnosed chronic medical condition.
