The fire inspection expired in November 2025. The program requested an inspection in October 2025 but was never completed by the Fire Department.
It was observed in TODDLER 1 that there was an unsecured electrical cord accessible to children.
It was observed in TODDLER 1 that children's allergy information was written on the white board. It was not posted in a privacy protected manner.
A review of five IHCPs indicated that three were incomplete.
It was observed in PRESCHOOL 2 that there were at least five children's water bottles that were not labeled for the child for whom they are intended.
It was observed in TODDLER 1 that there were food storage items in the handwashing sink.
Emergency Preparedness
The program did not follow its Emergency Preparedness plan in the event of a loss of heat or hot water. The local Board of Health received a complaint that the program continued to lack hot water after their HVAC system was replaced at the beginning of February.
This investigation inspection recorded no violations or advisories.
During the visit, first aid kits were reviewed. Two did not have band aids and one did not have a CPR mouth guard.
During the visit, it was observed in NURSERY 2 that there was chipped paint located on the wall underneath the crocodile activity board.

The attendance record in the NURSERY 2 classroom was reviewed and found to be inaccurate. One child was not signed in. This was immediately corrected by the Educators.

There was chipping paint and pieces of chipping plaster throughout the program. Please submit a timeline for correction and photos upon completion.

The following were observed: **KITCHEN: Accessible during visit- inside were gloves, wipes an apple corer, Clean Cide and the stove. **PS 2: gloves were accessible **PS 1: gloves were accessible **TOD 1: the grey rug was curling creating a potential trip hazard.

In two classrooms there were cords draping that were over 6 inches and were accessible to children. -In the infant room, there were 3 brown vinyl climbing circles stacked on top of each other. On top of these were toys. These could be tippable. -At the back door, an unmarked spray container was accessible to children. Program staff said it contained sanitizer. -In one room, a red/pink rug was curling, creating a potential trip hazard.
There was chipped paint and plaster in multiple rooms throughout the program.

The following were observed: -PS 1: A diaper changing pad was behind the trash can, touching the trash can. -T3: An easel was on a shelf and was tippable. The carpet was visibly soiled. -PS2: One shelf was tippable. The screw attached to it was not secured in the broken plaster. There was peeling paint behind the cots, which was exposed during nap time.

During the visit, it was observed in TODDLER 3 that the rest cots were not stored in a clean manner. They were located along the wall behind where the children were eating lunch. As the children were finishing their meals, they were observed touching the rest cots prior to washing their hands.
During the visit, it was observed in PRE-K that the blue soft chairs had tears on the arms.
During the visit, it was observed in NURSERY 2 that their fridge thermometer read above the 32-40 degrees range allowed for food storage.
It was observed that a bookshelf located in the hallway was not secured and tippable. The Program Director stated that this was removed from the Toddler 2 classroom for maintenance to secure properly.
It was observed that there was chipped paint and peeling plaster in the TODDLER 2 and PRESCHOOL 1 classrooms.
It was observed that children's allergy and special diet information was not posted in a manner that protects the privacy of each child in the TODDLER 3 and PRESCHOOL 1 classrooms.
It was observed that diapering and toileting procedures were not posted in the TODDLER 2, TODDLER 3, and PRESCHOOL 1 classrooms.
