The licensor reviewed 3 children's records with the following findings: 1/2 children have outdated enrollment/permission forms and an outdated annual physical; 1/2 children have an outdated transportation plan and developmental history.
The building inspection certificate is expired.
The licensor observed an inadequate amount of impact absorbing material in the fall zones of the large play structure.
The licensor reviewed the files of 3 children with a documented allergy/medical condition with the following findings: the required EEC IHCP for 1 child is not on file; a medical action plan is not on file for 1 child; the EEC IHCP for 1 child is not dated by the parent; the medical action plan for 1 child is not on official letterhead and does not have an official stamp by the health care professional.
The program does not have any documentation verifying the manufacturer's recommended age range for the large play structure on the on-site playground.
There isn't an adequate amount of impact absorbing material in the fall zones of the structure. There are rubber mats placed in the fall zones of the structure that pose a tripping hazard.
There is no evidence/documentation for an April 2025 staff meeting.

The licensor made the following observations while walking through the classrooms: the cabinet under the Preschool classroom sink is not secured and a spray bottle of Oxivir is accessible to children; a plastic storage cabinet in the bathroom across from the Preschool classroom has a drawer that is partially opened and contains a tube of Aveeno baby cream that is accessible to children; the bathroom used by the PreK children has a toilet brush that is accessible to children; a staff pocketbook is accessible to children on a counter in the Toddler 2 classroom; a toilet brush is accessible to children in the Toddler 2 bathroom and drawers accessible to children containing tubes of creams and plastic bags.

When observing in the Toddler 2 classroom the Licensor noted the following: 1. An Educator wiped a child's nose and did not wash her hands 2. The changing table was not washed and disinfected in between children being changed; the educator only changed the paper on the table.
