3 children's files were reviewed and the following was noted: 1. 1 child's file was missing evidence of an updated physical exam. 2. 1 child's face sheet was not signed/dated by the parent.

Several cords longer than 6 inches were found unsecured and accessible to children in the Discovery PS classroom.
When reviewing medication the following was noted: 1. 1 child's IHCP stated to administer [REDACTED] and the program did not have it on site. 2. 1 child's [REDACTED] box only contained the [REDATCED]t was missing the actual epi-pen.
This change of space visit inspection recorded no violations or advisories.

When observing in classrooms the following was noted: 1. Several shelving units were not secured and tippable. 2. A rocking chair was blocking the emergency exit in the Infant 2 classroom. 3. Children's feet do not touch the floor when seated at the table in the Toddler 2 classroom.

An IHCP was missing required information; Potential consequences if not treated and who has been trained to administer the medication. 2. 1 child's IHCP stated to administer only an [REDACTED]however, the child had an epi-pen and [REDACTED] at the program which was not listed as part of the plan.

Attendance was not accurate in 2 classrooms; Toddler 1, 9 children were signed in only 7 children were present due to 2 children transitioning into another room. PS 1 7 children present but only 5 children were signed in on the attendance sheet.

At the time of the visit the music teachers child was in the Toddler classroom, there was no evidence on site that the child was enrolled in the program.
2 staff files were missing evidence of a program orientation.
When observing in classrooms the Licensor noted there was not a sufficient amount of toys/materials independently accessible by the children, many were out of reach. More shelving units with toys need to be added.
At the time of the visit the following was noted: 1. Cords to the radio and phone were hanging down and not secured in the Pre-K classroom.
The gate leading into the kitchen was not closed so children had potential access.
When reviewing IHCP's the following was noted: 1. Several IHCP's did not have all required EEC information, they were missing the name of the Educators who were trained in the child's specific plan. 2. 1 child's plan was missing the parents' signature/date
Medications were reviwed and the following was noted: 1. 1 child's [REDACTED] prescription stated to discard after 12/11/24. 2. 1 child's [REDACTED] stated to discard after 11/30/24 and the [REDACTED]itself stated it expired 11/24.
At the time of the visit the following was noted: 1. Educator's purses/bags were stored on the floor accessible to children in the PS 2 classroom.
At the time of the visit the gate to the kitchen was open in both the PS and PK classrooms.
At the time of the visit the following was noted; 1. 1 child's prescription label for an epi-pen stated to discard after 8/2024. 2. 1 child's bottle of [REDACTED] had expired 7/2024. 3. A bag of medication was hanging on a hook within reach of children in the Pre-K classroom.
A Toddler 1 Educator sprayed the diaper changing pad with only a sanitizing solution; it must be cleaned first with a soap/water solution and then sprayed with a disinfectant (not sanitizer) and allowed to air dry.
