3 children's files were reviewed, and the following was noted: 1. 1 file was missing a transportation plan. 2. 1 file was missing evidence of an updated lead screening. 3. 1 file was missing the parents signature on the First aid emergency medical consent and release form.
3 staff files were reviewed, and the following was noted: 1. 1 file was missing an application or resume. 2. 1 file was missing an annual evaluation and professional development for last year and this year. 3. 1 file was missing a staff signature on the staff information form and professional development hours for this year. 4. 2 files were missing evidence of observations and consultations every other month.

2 children's IHCP stated to administer [REDACTED] for mild symptoms, there was no Benadryl on site for these children.
When reviewing medication the following was noted: 1. 1 child's [REDACTED] prescription had expired, it stated to discard after 3/4/2024. 2. 1 child's [REDACTED] was not in its original container or with the original prescription label affixed to it. 3. The backpack with [REDACTED]inside of it was stored on the floor accessible to children in the PS 1 classroom, it must be stored up high out of reach at all times.
At the time of the visit the following was noted: 1. A spray bottle of bleach/water was found stored on a low shelf accessible to children in the Toddler 3 bathroom. 2. A box of trash bags was found stored on the floor accessible to children in the Toddler 3 bathroom. 3. A surge strip was found without plastic safety covers in the PS 2 classroom.
At the time of the visit written emergency evacuation procedures and diagram of escape routes were posted however they were not posted by each means of exit in every classroom.

When reviewing childrens files 1 child did not have evidence of a progress report every 3 months when they were an infant, then every 6 months as a toddler.
6 children's files were reviewed and the following was noted: 1. 1 child's Transporation plan was incorrect it stated the child arrives and departs by program bus/van and the program does not offer transportation. 2. 1 child's Developmenatl history was not signed/dated by the parent so it could not be determined if it was valid.
6 staff files were reviewed and the following was noted: 1. evidence of the verification of 2 verbal references was missing from 4 files. 2. An annual evaluation was missing from 1 file. 3. Evidence of a program orientation was missing from 1 file. 4. Evidence of First Aid training was missing from 1 file. 5. Observations and consultations with staff every other month were missing from all files.
At the time of the visit there was not a sufficient amount of toys/ equipment in the Toddler 1 classroom. Several shelves were empty.
At the time of the visit children's blankets and sleeping bags were in piles touching one another in several classrooms, this is not considered safe and sanitary storage.
When reviewing IHCP's the following was noted:1. 1 child with asthma did not have a IHCP on file. 2. An Educator reported a child had [REDACTED] but stated there was no [REDACTED] on site for this child, after speaking to the Director it was learned this child did have an inhaler in the classroom backpack. 3. 1 child's IHCP did not state which Educators were trained in their plan.

At the time of the visit the Licensor noted the following: 1. Diaper creams were found stored in the bottom of the changing table, accessible to children in the Toddler 2 classroom. 2. Swiffer Wet Jet Mops were found leaning against walls in several classrooms.
3 children's files were randomly reviewed, and the following was noted: 1. 1 file had forms that expired 12/2022; Face Sheet, 1st Aid Emergency Consent and Release form, Transportation Plan and Developmental history and this child did not have a progress report every 3 months as required for infants.

2 staff who have been employed longer that 6 months did not have evidence of First Aid training on file as required.
3 staff files were randomly reviewed, and the following was noted: 1. 1 file was missing the documentation of interview date, evidence of the verification of 2 verbal references, evidence of a completed BRC, evidence of professional development hours, evidence of a program orientation and staff observations and consultations every other month. 2. 1 file was missing evidence of the verification of 2 verbal references, evidence of a completed BRC, evidence of professional development hours, evidence of a program orientation and staff observations and consultations every other month. 3. 1 file was missing the documentation of interview date, evidence of the verification of 1 verbal references, evidence of a completed BRC, evidence of [REDACTED] evidence of professional development hours, evidence of having completed the EEC 12 Essential trainings, evidence of a program orientation and staff observations and consultations every other month.
The PS 3 classroom attendance was inaccurate, 17 children were signed in but only 16 children were present. One child was signed in who had not arrived yet.
When observing in classrooms the following was noted: 1. Brooms were found not secured leaning against walls in several classrooms. 2. A lamp was found on a high shelf with the cord hanging down and plugged in below, the cord was not secured to the wall. 3. A water dispenser that dispenses hot water was found in an are accessible to children.
At the time of the visit the following was noted: 1. 2 chairs were found on top of 2 children's tables in the Gross Motor room while children were playing in it. Once the Licensor brought it to the Educators attention, she removed the chairs immediately. 2. A tall black metal shelf (next to the easel) was not secured posing a potential hazard in the PS 2 classroom.

When observing in the Toddler 1 classroom the Licensor noted 16 empty shelves. There was not a sufficient quantity or variety of materials and equipment to engage all children present in the Toddler 1 classroom. The materials and equipment was not visible and readily accessible to the children in care and was not arranged to promote independent access by the children.
At the time of the visit the following was noted: 1. Diaper creams and petroleum jelly was found accessible to children in the Toddler 3 bathroom, CeraVe cream was found in a cubby in the Toddler 2 classroom and Hand sanitizer was found on a low shelf in the gross motor room.

At the time of the visit for the new space the following was noted: 1. wooden cubbies were not secured and posed a potential tipping hazard 2. Gates need to be added to doorways to separate classrooms.

When observing in the "Toddler 2" classroom the Licensor noted Educators did not provide guidance to children in a positive and consistent way based on the understanding of the individual child's needs and development. The Educators did not encourage self-control and did not use positive child guidance techniques such as recognizing and reinforcing children's appropriate behaviors, having reasonable and positive expectations, setting clear and consistent limits and redirecting. Educators were observed using the words "no", "out","sit down" "don't", "you're not " without using any positive guidance techniques or redirection. One child was observed crying and wandering the room, after approximately 20 minutes minutes "Educator A" told him to "go play" the child shook his head no and "Educator A" walked away, the child continued to wander and cry. A child was building with stackable blocks, "Educator B" said to the child "it's too high", reached over and took the top of his structure off, without any warning or discussion. "Educator B" was seated at the lunch table and she stated in a stern voice to a child "you are not playing puzzles, you are going to bed", pointing with [REDACTED] finger in the direction of where the child's mat was.

Of the 2 staff files randomly reviewed the following was noted: 1. Both files were missing evidence of a program orientation prior to working with children, staff observations and consultations every other month. 2. 1 file was missing evidence of registration in the EEC PQ Registry and an annual evaluation.
The staff schedule hanging on the wall was not current/accurate.
The “PS 1” classroom attendance was inaccurate; 8 children were signed in and only 7 children were in attendance.
An electrical strip outlet in the “PS 1” classroom did not have safety covers plugged into all open outlets.
From review of the evacuation drill log it was learned emergency evacuation drills were not practiced monthly as required.
[REDACTED] stated to administer [REDACTED]which is considered an emergency medication; there was none on site for the program to administer.
At the time of the visit the following was noted: 1. The “Toddler 2” table was washed with bleach/water first, then soap/water.

The Licensor asked "Educator B" where the [REDACTED] was and "Educator B' stated it was in the office. When the Licensor asked the [REDACTED] it was learned there is no medication on site because it is a 2 time a day medication and is given at home per [REDACTED[ 2. 1 child had 2 IHCP's on file that did not match; The Dr.'s plan states to administer Benadryl and an Epi-pen however the plan the parent's filled out said to administer only an Epi-pen.
