6 staff files were reviewed and the following was noted: 1. 2 files were missing evidence of an updated physical exam. 2. 1 file was missing evidence of [REDACTED] and evidence of updating in the PQ registry. 3. 1 file was missing a completed staff information form, physical exam and [REDACTED]evidence of registering in the PQ registry. 4. All files were missing evidence of staff observations/consultations every other month (the program was doing them every 3 months).

4 Staff files were reviewed and the following was noted: 1. The Site Coordinators file was missing evidence of qualifications for the position (Education and work experience), evidence of a completed BRC, evidence of a physical exam/MMR, evidence of having completed the Strong Start trainings and evidence of updated First Aid training. 2. 1 staff file was missing evidence of qualifications for the GL position (Education and work experience), evidence of a completed BRC, evidence of a physical exam/[REDACTED], evidence of having completed the Strong Start trainings and evidence of updated First Aid training. 3. 1 staff file was missing evidence of a completed BRC, and physical exam/[REDACTED]. 4. 1 staff file was missing date of hire, evidence of a completed BRC, physical/[REDACTED]
At the time of the visit there was no evidence of fire evacuation drills being completed monthly as required. The log could not be located.

At the time of the visit there was no evidence that progress reports were completed for the 2021-2022 school year.
6 children's files were reviewed and the following was noted: 1. All required documents in files had expired; they stated they were last updated in 2019 and 2021, parents signatures/dates were missing (there is no place on the form) and nothing stating a physical exam and immunizations are kept on file at the school (this is also not on the forms).
At the time of the visit the Licensor noted injury reports in children's files however there was no documentation in the injury log. The last injury documented was 9/2021.
At the time of the visit 1 child with an IHCP and emergency medication was listed on the allergy/emergency medication posting however his medications [REDACTED] were not listed.
When reviewing medication the following was noted: 1. 2 children with Benadryl listed on their IHCP as an emergency medication did not have any on site at the program. 2. 1 child's [REDACTED]l which was listed on their IHCP as an emergency medication was not readily available as it was kept upstairs in the cafeteria/office area.
