Licensor observed all children's records did not contain photo or ID information.
Licensor observed children's belongings stored in backpacks on the floor in the basement near the stairs. Licensor did not observe labeled/assigned storage for children's personal belongings. The educator stated (11) children are enrolled.

Educator using backyard, no hazards observed in the yard during the visit. Licensor observed (2) children require life saving medication. The licensor observed that the educator did not have an IHP list posted.

No violations observed outdoors during the visit. The licensor observed an unsecured child sized kitchen set in the basement. The licensor observed yoga mats used by children were not assigned/labeled. The licensor observed the carbon/smoke detectable is operable. The licensor observed a drill. The licensor observed an air purifier in operation during the visit . A gate prevented access to the first floor level. Windows appeared in good repair.

The Educator did not have the children wash hands with liquid soap and running water before or after eating. The Educator also did not have individual towels or disposable towels available for the children to dry their hands.

One child is missing evidence of a [REDACTED]. Three children’s records are missing all or part of the Physical Description. Two children are missing the day of Admission. And one child’s schedule of days and times in care is not accurate.
The First Aid kit was missing life saving medications for two children who require them. The First Aid kit had a thermometer but it was not working.
Two children are missing Individual Health Care Plans who require them.
The Emergency postings are not visible to parents or visitors, upstairs.
The Educator did not have a list of emergency or life saving medication posted visibly with a cover sheet for the children who require it.
The Educator was observed allowing the children to sit and eat at the table that had not been washed or disinfected beforehand. The Educator did not wash or disinfect the table after the children had eaten.
The Educator was unable to demonstrate that the Carbon Monoxide Detector was in working condition.
There were hazards in the kitchen: knives, peelers, and a pizza cutter in a drawer, tin foil, Saran Wrap, and plastic bags in a drawer, sharp equipment in the lazy Susan, chemicals in the cabinet, an outlet that did not have outlet covers, and wine bottles by the kitchen table. The upstairs bathroom had chemicals in the cabinet. There was an open chest in the Basement Play room that has plastic bags that store toys for the children. All of these hazards were accessible to children.
The Emergency Exit in the Basement is missing an Exit sign on the last door that opens to the Bulk head stairway. The Emergency path to the Exit was obstructed by toys and equipment. The Exit stairway does not have a child sized railing.
The Educator was observed changing diapers, without using a protective barrier in between the plastic changing pad and the child. The Educator did not disinfect the changing pad in between diaper changes. The Educator did not disinfect the diaper pad after she washed it with a wipe. The Educator did not dispose of the diapers in a covered, lined container. Neither the Educator nor the children washed their hands after diapering.
