An infant was asleep in the top crib and the small rail was not in the upright position/locked into place.
Sleeping bags in the Pre-K classroom were piled inside a cubby area all touching one another.
A plastic Step 2 climber/slide did not have the required impact absorbing material in the 6-foot use/fall zone completely surrounding it.
The diapering plan was not posted in the PS/PK bathroom where the changing table is located.
Written emergency evacuation procedures were not posted by the Toddler room main door and the upstairs PS/PK classrooms main door. They should be posted next to the diagram of escape routes.

2 staff files were reviewed, and the following was noted: 1. 1 Educator was missing evidence of an updated physical exam., 2 [REDACTED] and the EEC Essentials trainings. The staff information form was not signed/dated by the Educator.
This investigation inspection recorded no violations or advisories.
At the time of the visit the following was noted: 1. Hand sanitizer was found on a low shelf accessible to children in the PS room and Lysol spray disinfectant and Clorox wipes were found on a low shelf accessible to children in the Pre-K room.
