2 staff files were reviewed and the following was noted: 1. 1 file was missing evidence of the verification of 2 verbal references, BRC suitability letter, and evidence of registration in the PQ registry. 2. 1 file was missing BRC suitability letter, updated physical exam and updated PQ registry.
The Licensor noted cords to clocks, a router and TV that were not secured to the wall posing a potential hazard.

At the time of the visit the staff schedule posted was not current, one Educator is no longer employed at the program.
At the time of the visit a 13 year old child (an Educator's son) was in the program but the Licensee stated he was not enrolled. All children must be enrolled in the program in order for them to be on site during program hours. 6 children's files were reviewed, and the following was noted: 1 The Licensee's child did not have a file on site. 2. Date of Admission was missing from all files. 3. First Aid/Emergency Medical Consent & Release forms were missing from all files. 4. 1 child did not have a transportation plan.

At the time of the visit snacks were not served in a safe manner; children were observed sitting on the floors eating.
Toxic substances such as hand sanitizer, Lysol Disinfecting spray and Clorox wipes were found stored within reach of children.

At the time of the visit there was no documentation of staff meetings on file to review.
5 Staff files were reviewed and they were all incomplete; they had not been updated since the non-compliances found at the pre-licensing visit. The Licensee submitted a corrective action plan stating all files were complete by October, 14, 2022.
At the time of the visit there was no injury log to review; the Licensee stated they have not been writing injury reports.
When the Licensor arrived attendance was not accurate, the program had 20 children present and only 17 signed in.

At the time of the visit the log was not available to review as the Licensee stated they were not practicing evacuation drills.
At the time of the visit the following was noted: 1. 3 IHCP's did not list the names of the Educators who were trained in each child's IHCP. 2. 1 child's plan did not include what treatment was necessary at the program if the child accidentally ingested gluten.
At the time of the visit one child had an IHCP on file that stated he needed an [REDACTED]and there was no [REDACTED] on site.

At the time of the visit children's records were not on site to review
At the time of the visit, Staff records were not on site to review
Attendance was not accurate, and Ratios were not being maintained at all times. 1. One Educator walked down the hall and out of the building when he saw the next van of children were beginning to arrive, leaving 6 children alone/unsupervised in the main classroom. The driver was at the end of the line and had not entered the building yet. 2. One Educator was alone with 15 children as children were being dropped off at the program. 3. 1 Educator left the main classroom leaving the other Educator alone with 24 children. 4. Attendance was not taken as children entered into the program. The Licensee/Director took attendance and it was not accurate; 20 children were present and only 19 were signed in. 2. Actual arrival and departure times were not being recorded on the attendance sheet.

At the time of the visit the Licensor noted 2 Educators supervising a group of 24 children, one of those Educators was busy with 3 children on the computer and not appropriately supervising the group. Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children

At the time of the visit a [REDACTED] was doing community service at the program. This volunteer did not have a current BRC on file and was allowed to take a group of 4 children down the hall to the bathroom alone.
4 girls were allowed to change their clothing in one bathroom unsupervised. The educator may only allow one child at a time to use the bathroom unless they are being supervised.
Written evacuation procedures and diagram of escape routes were not posted as required.
At the time of the visit the program was not following infection control or hand washing procedures: 1. Tables were not washed with soap/water and sanitized prior to snack. 2. Children did not wash their hands prior to eating snack. 3. A child was observed not washing his hands after using the bathroom, the Licensor had to remind him. 4. Children were observed sitting on the floor eating snack.
