While in the Preschool room, Licensors asked about emergency medications in the room and IHCP's. The teacher (who was filling in for another staff at that time), stated she did not know where the medications were, or if any children in the room had an IHCP.
1.) In the Preschool room, one of the children was helping put out napkins and other items for snack. The child did not wash their hands before handling these items. 2.) In the Preschool room, the teacher washed the tables with soap and water, but did not disinfects the surfaces. 3.) In the Toddler 1 room, the teacher disinfected the changing pad after diapering, but did not clean it first. 4.) In the Crawlers room, a child's hands were not washed immediate after diapering. The child was put on the floor and started playing with toys before the teacher brought the child to the sink to wash hands. 5.) In the Crawlers room, while sanitizing the changing pad after diapering, the teacher sprayed the cleaner and the disinfectant on the pad at the same time.

Three staff records were randomly selected for review and the following was learned: 1) Three staff did not have evidence of bi-monthly observations as required by regulation.
During a health and safety review of the program's indoor space, the following was learned: 1) there were stained ceiling tiles in several spots, creating a potential hazard 2) not all changing tables were secured as required by regulation 3) In "Pre-K" the blue toy shelf and the kitchen set were not secured as required by regulation 4) in the "Pre-K" room the bottom of the room divider has fallen off and is on the floor of the classroom, creating a potential hazard.

During a review of the program's indoor licensed space, the following was learned: 1) Not all equipment, including shelves, kitchen sets and diaper tables were secured as required by regulation.
Although the Director reported that the inspections were scheduled for later this month, the program did not have evidence of a current building inspection or fire inspection are required by regulation.

During a review of the program's IHCP forms, the following was learned: 1) Child A did not have evidence of a current IHCP form as required.
During a staff record review it was learned that two educators did not have two documentation professional references as required.
During a review of program's medication, it was learned that Child A did not have current consent for their emergency medication as required.
During a review of the program space, the following was observed: 1) throughout the program it was noted that toys shelves were not secured as required 2) toys shelves were observed to have chipping paint and, in several classrooms, including "toddler 2" there was chipping wall board, accessible to children. 3) a stained ceiling tile was observed in "pre-K" creating a potential hazard to children 4) items were observed to be stored in the emergency evacuation crib in "crawlers" creating a risk to children
Medication
During a review of the "toddler two" space, it was learned that the diaper table is not adjacent to water as required.
Staff interact with and assist children during snack and mealtimes.

In the "preschool one" classroom the educator was observed to serve the program offered snack to children but placing it directly on the table in front of them without a barrier, creating a potential risk.

During a review of the outdoor playground space, the following was learned: 1) On the preschool side, there is an entrapment gap under the gate (that leads to the field space) 2) On the preschool side, there is an entrapment hazard under the shed, creating a potential hazard.
Building Inspection and Fire Inspection - R Apr 2025
Three staff records were randomly selected for review and the following was learned: 1) two staff did not have evidence of two professional references as required by regulation. 2) one staff had evidence of only one professional reference 3) one staff did not have evidence of completing the 12 hour "Strong Start" training 4) one staff did not have evidence of a current PQ registry.
During the visit, it was learned that the program did not have evidence of consistent 2-hour monthly staff meetings as required by regulation. Additionally, it was learned that the program had not conducted a staff meeting since October 2023.
