At the time of the visit the following was noted: 1. Attendance was not accurate in the PS U classroom; 11 children were present and only 10 children were signed in. 2. Teacher: Child ratio was not appropriate; 1 Educator was alone with 10 children.
1 Educator in the Infant D classroom wiped a child's nose and did not wash her hands.

Indoor Space
At the time of the visit there was a crack and a hole in one floor tile inside the Pre-K mudroom, and it appears that the tiles are separating from one another.
Outdoor Space - R Apr 2025
When observing the PS playground, the following was noted: 1. The plastic coating on the edges to the stairs of the PS climber, and platforms is worn off and metal and rust are exposed posing a potential hazard. 2. There was not 9 inches of measurable wood chips in the use/fall zones surrounding the climbers and swings. 3. A metal shade structure was located within the use/fall zone of the swings.

3 children's files were reviewed, and the following was noted: 1. 1 file was missing evidence of an updated physical exam.
1 child's prescription for an Epi-pen stated to discard after 3/29/23 and the actual epi-pen expired 4/23.
1 Educator in the Infant D classroom did not wash the child's hands prior to feeding the child snack.
1 child's IHCP had expired. 4/1/2023.
When observing diapering in the Infant D room the following was noted: 1. 1 child's hands were not washed after diapering. 2. The diapering surface was not washed with soap and water prior to the Educator spraying the disinfectant solution on.

When observing diapering the following was noted; 1. An Infant A's child's hands were not washed after diapering. 2. The Toddler H classroom only washed the diapering surface with sanitizer, the surface must be washed with soap/water or cleaner and then a disinfectant.
