1) Chipping paint was observed in several areas of the building, accessible to children. 2) The door leading to the hot water tank closet was unsecured in the "Krusaders" room, creating a potential hazard to children.
Several of the bolts on the railroad ties were observed to have pulled up, creating a potential hazard.
1) Child B had [REDACTED] the expired 4/20/24 2) Child B had a written prescription for [REDACTED]that was not current on the box as required.

During an observation of the indoor space, the following was observed: 1) there was a missing outlet cover in the preschool room 2) chipping paint was observed in the downstairs "krusaders" bathroom and on several shelving units in the same room, creating a potential hazard to children 3) the door leading to the basement storage and hot water tank was found to be unsecured, creating a potential hazard.
During a review of program IHCP, the following was learned: 1) Child A did not have a current IHCP or medication consent form as required by regulation.
Not all equipment was secured as required by regulation.
Three staff records were randomly selected for review and the following was learned: 1) one educator did not have evidence of having provided the program with a current physical as required 2) One educator did not have evidence of having current first aid certification within 6 months of their hire date as required by regulation.

During the indoor assessment of the program the following was learned: 1) chipping paint, accessible to children, was located throughout the classrooms, creating a potential hazard. 2) Not all electrical cords were secured as required in the downstairs PS room, creating a potential hazard.
Chipping paint was observed in at the window still in the "Small Fries" classroom and in the shared children's bathroom upstairs.
During a review in the "Little Rascals' Classroom (PS) it was observed that children did not immediately wash their hands prior to eating snack.

Diapering and Toileting for children under 2.9
During a review of Child A's IHCP form it was learned that the form did not contain all EEC required information, including the side effects of the medication and potential consequences if the medication was not given.
Wet Diapers were not placed in a closed container that is lined with a leak-proof disposable lining in the children's bathroom space as required by regulation. As a result the bathroom smelled strongly of urine.
It was observed that several sections of the wooden fence were cracked, creating sharp edges. There were 2 sections of the fence gate that was broken and did not close, creating a potential hazard. The rail road tie that surrounds the sandbox on the SA side was observed to have bolts pulling up, creating a danger. Some of the playground equipment had been moved, and placed in the fall zone of other equipment, creating a hazard. The hose was left on the ground on 2 playgrounds, creating a tripping hazard.

During the visit, it was observed that the [REDACTED] assigned to the preschool grouping was on her cell phone when she was assigned to be supervising children. The Assistant Educator in the toddler room was observed to take children to the group bathroom for diapering, without being under the direct supervision of a Teacher certified person as required by regulation.

During a review of program medication, it was learned that not all non prescription medication has a form that has been signed by the doctor that gave the order. Not all medical consents were found to be current. A [REDACTED] store brand medication at the program.
