There are a few staff that recently returned to the program and will be working with the YMCA through the summer. However, the program did not have any staff records for these individuals.
Licensor reviewed 3 of the children's records and they all had expired signatures for the authorizations and 2 of these records also had out of date physicals. Please review all children's records and ensure they are all complete and up to date.

There were no fire drills documented for April, May or June. Discussed that regulations state a fire drill must be conducted at least once per month.

Three Staff records were randomly selected for review and the following was learned: 1) Educator A did not have evidence of having provided the program with a current physical or MMR records as required.

While children were eating snack, it was observed that there was not a barrier between the snack and the table as required.
During an observation of the program's cleaning/sanitizing the following was observed:1) the snack table was not washed/sanitized prior to children eating snack
Emergency medications readily available
Not all IHCP forms were found to be current (signed in the last year) as required by regulation.
1) While the program did have all emergency medications on hand, the back pack containing those medications was observed to be stored in the reach of children when the back packs was left on the table in the cafe. 2) Some of the emergency medications were found to be expired.

During a review of medication, it was learned that Child A has a "allergy action plan" that expired 3/8/22 that was also not signed by their parent as required.

2 staff records were selected for review and the following was learned: Educator A did not have evidence of a staff form, application/resume, references, documentation of EEC qualifications, a physical/[REDACTED]program orientation or "Strong start" trainings as required.
A current staff schedule was not posted where staff, parents and visitors can review as required.
On the day of the visit, the program did not have a current building or fire inspection available for review as required. The program did not have evidence that the "location of the health care policy" is posted as required.
During a review of the fire drill log, it was learned that in the 2021-2022 school year, only 2 drills were conducted and documented. The program is required by regulation to practice and document fire drills monthly.

During the visit, it was observed that the Site Coordinator sent the Assistant Group Leader to meet the bus. The Assistant Group leader was then observed to be left alone in the room when the Site Coordinator left the room to collect supplies in the kitchen next door.

Three children’s records were reviewed and the following was learned: 2 children did not have evidence of a current progress report.
It was observed that in one window sill the screen was pulling out, creating a potential hazard. In the second window a wooden piece has pulled away from the frame, creating a potential hazard.
There was not evidence to support that the program has conducted monthly fire drills since it reopened in August 2019 as required by regulation.
During the visit, it was observed that paper towels were stored in an unsanitary manner, when the roll of "brown" paper towels was left next to the sink for children to wash their hands. The first child that ripped off the paper towel was observed to get the entire roll wet, making it difficult for the remainder of the children to use the paper towels or dry their hands.
