During a review of all staff BRC, the following was learned: 1) Educator A had been run under the previous program number and not the current program number as required 2) Educator B had a BRC that had been run under the previous program number and was working in the program without being granted "provisional" status as required.
During a review of three staff records, the following was learned: 1) Educator A did not have evidence of a current [REDACTED] records on file as required 2) The Licensee had evidence of one[REDACTED]required by regulation.
One staff file did not have evidence of a physical exam.
