The staff meeting documentation was missing the months of March, April, May and they did not include the times of the meeting.
A review of five staff files, Licensor noted the following: 1.One educator recently hired was missing evidence of date of interview, verification of 2 references, MMR immunizations, physical examination and program orientation. 2. One educator recently hired had a blank staff form on file, was missing date of interview, verification of 2 references, physical examination, MMR immunization, and EEC Strong Start Essentials 12 hour certificates. 3. One educator recently hired was missing date of interview, verification of references, physical examination, MMR immunization, strong start essentials, TQ qualifications, PQR, and program orientation. 4. One educator was missing evidence of MMR immunizations, program orientation, staff observations, and an annual evaluation was dated 7/2020, PQR expired 12/7/23. 5. One educator was missing evidence of a physical examination MMR immunizations, and program orientation.

A review of four children files, Licensor noted the following: 1.One child first aid medical consent form and transportation form were expired dated 8/5/23. 2. One child had a physical examination dated 2/16/23. 3. One child first aid medical consent form, transportation form, off site permission form were dated 7/26/23.
A review of files found three children missing progress report as required every three months for infants and every six months for toddlers and preschool.

The licensor noted the following: Preschool: A small section of flooring was lifting and not secure. This was especially noticeable at a seam in the interlocking flooring system connecting the cubby area to the main classroom. The lifting posed a potential tripping hazard to children. All shelving and cubbie units were not secured from tipping hazards. A bulletin board was not secured to the wall and placed on Top of a cubbie against a wall posing a tipping hazard to children. There was also a wipe off board in the main hallway outside of the preschool room on top of a shelf and was not secure form tipping. Preschool bathroom: Some plumbing pipes did not have foam tubing covering the pipes. Please cover all pipes so that children do not have access to the pipe itself. The toddler room had built in cubbies that were made of wood and the edges on the right side were splintered and had chipping flaking paint.
All classrooms did not have the written evacuation plans posted by all means of egress.
