Safe Sleep
It was observed at time of visit that there was a (REDACTED) sleeping in a pack and play with a boppy pillow and swaddled in a blanket. Educator removed pillow and blanket immediately. Safe sleep regulations and policy were reviewed with the educator.

Diapering table was observed to be easily moveable
Attendance records were reviewed. It was observed that on two separate dates there were two children signed in, however the date was not documented. There was one day in July that a child was not signed in at time of arrival only signed out.
Educator has a personalized parent handbook and also utilizes EEC’s parent handbook for private enrolled children. EEC’s parent handbook could not be reviewed, educator did not have a hard copy on hand. Educator will need to upload parent handbook to her document library.

Emergency exit from first floor slider door was observed that there was a kitty pool setup and was filled with water. Discussed with the educator that she must ensure all emergency exits are clear from hazards. Se observo que la piscina estaba llena de agua lo cual es un peligro. Las salidas de emergencias deben estar seguras en todo momento.
Educator is documenting children’s arrival and departure times; however has not been documenting her assistant’s daily attendance. La educadora no ha estado documentando la asistencia diaria de su asistente.
Educator indicated that she had used her deck with children to use kitty pool; deck is not approved for use with children only as a means of egress. It was discussed with the educator that if she would like to use the deck as additional outdoor space, barrier around deck must be at least 4 feet tall. La educadora indico que uso su deck para juego con los ninos, para uso de piscina pequena. El deck no esta aprobado para uso de juego solo para salida de emergencia. Si la educadora quiere que el deck se apruebe, la barrera alrededor del deck debe medir minimo 4 pies de altura.

There were no trainings registered in the educator’s PQ registry. Educator did not have on hand proof of trainings she indicated she has taken. La educadora no ha registrado sus entrenamientos en su PQ y no tenia copias de sus certificados de entrenamientos que indico que tomo disponibles.
CPR expires on 9/2022 First Aid certification could not be verified, educator will need to send proof of up to date first aid. No se pudo verificar la certificación de primeros auxilios.
It was observed that the outdoor play area was still being worked on; the fence was not completely enclosed and there were access to large wooden boards. Se observo que el area de juego en la yarda no estaba completamente lista. La barrera no estaba completamente instalada y habia acceso a pedazos grandes de madera.

A random review of children’s files was completed. It was observed on file had a blank general permission.
Deyanira Ramirez certified assistant for 6. Martha Arias certified assistant for 6 did not have current physical and did not have proof of ongoing professional development training.
Emergency preparedness plan could not be reviewed, educator did not have a hard copy of her parent handbook
It was observed that the railing from the second emergency exit is too high for children to be able to hold on to properly.
It was observed that the climbing structures did not have the 6 foot distance from each side as required. It was also observed that the ground surfacing did not have the required amount of rubber mulch, which should be 9 inches.

It was observed that there was no barrier to prevent access to stairways on one side of the basement level playroom. No habia una barrera puesta para prevenir el acceso a las escaleras de un lado del basement.
Refrigerator temperature could not be read due to refrigerator not being on at the time of visit. La temperatura del refrigerador no se pudo verificar ya que no estaba prendido al tiempo de la visita.
