The licensor arrived at the home for a visit, and the Educator's husband answered the door, holding a child in his arms. He informed the licensor that the Educator was closed for the day and stated that the child he was holding was his nephew, while the infant crying inside the home was his niece. The licensor entered the house to confirm that these were the only children present and discovered the Educator in a bedroom with three other childcare children. Although the Educator was open and caring for four children, she did not want the visit to occur, so she attempted to make it seem as if she was closed. She instructed the household member to provide false and misleading information to the Department to avoid the licensing visit.

The licensor reviewed 6 enrollment packets and found: 3 blank general permissions, 2 date of admission blank, 2 child description blank,3 children did not have a medical and 1 child needs an updated medical
The educator did not have the children wash their hands before eating lunch. There was work being done in the bathroom and it was not safe for children to be in there without adult supervision
The Educator did not demonstrate the (REDACTED) to care for the children when she did not follow a doctor's orders/note that required her to be out of work until October 21, 2024.
Although the back door will open/close when you pull on the area with the screen the handle is missing
The licensor observed 2 cords accessible to the children on the white shelf in the bedroom The backyard had a lawnmower accessible
When the licensor arrived the children were not engaged in activities. 3 of the children were in the back bedroom with the educator with the door closed and no activities
The diaper table has a cloth cover over it preventing it from being disinfected after use
