A review of staff records, Licensor noted the following: 1. Two educators from another location were working at the program and did not have a file on site available for review. 2. One educator was missing evidence of MMR immunization, application/resume, date of interview, and verification of 2 references. 3. One educator was missing verification of 2 references and evidence of MMR immunizations.
Progress reports were not available for review.

A review of three staff records indicated the following: -Three staff did not have evidence that they completed an orientation. -Two staff did not have evidence of a current physical examination. -Two staff did not have evidence of immunity to MMR. -One staff did not have bi-monthly observations. -1 staff did not complete their EEC Strong Start trainings. -Three staff did not have a application or resume. -Two staff were not registered with the Professional Registry. -One staff working in the program on the day of the visit did not have a staff record available at the site for review. A review of six children's records indicated that all of the records did not have enrollment forms signed by the parent/guardian.

Emergency procedures were not posted in the Library or in the Cafeteria's secondary exit door.
A review of six children's medication indicated the following: -One child enrolled in the program did not have an (REDACTED) as they required for a severe allergy. -Two children enrolled in the program that had(REDACTED) did not have (REDACTED) at the program. -Two (REDACTED) were not stored at the program in their the original packaging with a prescription label. One of these(REDACTED) did not have an expiration date on the medication. -One child who required (REDACTED) as part of their Individual Health Care Plan (IHCP) did not have any at the program. -An empty pill bottle that previously contained (REDACTED) was being stored with the current medications. Please dispose.

In the Cafeteria three cleaners were being stored on a table next to children engaged in an activity. The cleaner bottles were marked with "Keep Out of Reach of Children".
