The educator is not completing progress reports on a regular basis. The last progress report was completed on 12/18 for all children in care
The educator did not have attendace for the day of the visit. The last documented attendace was on 10/4/19. Additionally there are missing weeks in the attendance records where attendace is not documented. The educator is not always including [REDACTED] on her attendace
The backyard has hazards including but not limited to: nails, wood, electrical cords, power tools and lots of debris from the deck that is currently under construction and has been since 9/30/19. The Educator provided written notification to parents on 9/3019 As of today’s visit the backyard is not approved for use

The following hazards were observed : bathroom: air freshener was accessible and plugged into the outlet at the sink, there were pipes accessible where a washer/dryer should be. There is a plug in air freshener plugged in and accessible in the playroom. There are 3 shelves and a diaper table not secure to the walls in the back room.
There is a metal stove in the middle room behind the cubbies that is accessible. The heating vent in the playroom was broken and accessible to the children. The radiator in the kitchen was accessible and needs to be made inaccessible
The Educator’s front steps had chipping/peeling paint
The licensor observed the following in the backyard: the fence was broken in 3 areas: 2 of the areas lead to the street and 1 leads to the above ground pool.Yard cannot be approved until this is fixed.
There are 2 boards that need to be replaced on the deck, one is broken and rotted and one is loose.

Educator's outdoor space only has concrete surface, discussed adding and extending area where there was some mulch in order to have a softer surface for children to play. Outdoor space did not have the required enclosed fencing; educator lives in a busy street. Indoor space did not have outlets covered. There was access to some cleaning products and beauty products under the bathroom sink.

Space was not setup with materials and furnishings. A follow up visit will be conducted once space is completely setup.

Safe Sleep
There were 6 children on the day of the visit. A review of the attendance records shows that on 9/30/19 [REDACTED] was not signed in on the attendace until 4:30pm. According to the educator[REDACTED] was present at 11:30am for[ REDACTED], that would make 7 children present and none of them are school age
The educator went upstairs to get lunch for the children in care, leaving the 5 children downstairs unsupervised. The [REDACTED] was napping at the time. The children were ages [REDACTED]. The lunch was prepared and kept upstairs, the educator has been cited for this in the past
The licensor observed peeling paint in the backyard on the gazebo which is in licensed space
The licensor observed the educator to place the [REDACTED] down in a pack and play and the sheet was not tight fitting.
