A random review of children’s files was completed. Files had signed general permission to take children off premises, however the general permission does not specifically indicate where children are taken. One file had the parent contact information during daycare hours blank. One file had the emergency card missing parent signature and signature date.

It was observed that the educator went up to the first floor for about a minute to grab a dish. Educator cannot be going between floors and must remain on same floor level as all children.
It was observed that for some of the children in care daily attendance did not reflect actual arrival and departure times.
Children’s records were reviewed. It was observed that three files did not have physical description of child. One file had missing lead screening results. The general permission for all files did not indicate specifically where children are taken off premises and two files did not have parent signature and date for general permission. Four files did not have the acknowledgment of receiving parent handbook signed and/or dated. Two files had varies pages in the enrollment packet that were not signed or dated by parent.
