Children’s records were reviewed. One file was missing date and age of admission. Two files were missing physical description section and the parent signature and signature date for acknowledgment of receiving the parent handbook. One file did not have annual update. One file did not have an updated physical. Three files had general permissions missing exact information where children are taken when off premises and two had parent signature indicating that they did not give permission to take children off premises; educator takes children for neighborhood walks and needs to ensure parents give permission.
