The daily attendance records were reviewed. The Educator did not sign the children out of care on 3/30/26 and 3/31/36. The Educator has been cited for this multiple times.
There was not a distance of at least 2ft between the nap materials today or a distance of at least 3ft between children’s faces The educator was given technical assistance regarding this at a visit on 3/11/2026
A (REDACTED) was observed to be sleeping in an infant seat on the day of the visit. The assistant removed the infant from the infant seat and stated the infant had been sleeping in there for a few minutes. The pack and play was not set up for the infant to nap in, there were toys inside it, the sheet was in the dryer and the mattress was not installed properly. This is the Educators second citation of safe sleep.

The Educator’s attendance records for the day were not accurate. A review of the day’s attendance showed eight children signed in, there were 10 children present during the visit. The Educator completed the attendance while the licensor was present.
This follow-up visit inspection recorded no violations or advisories.
Exits (evacuation route)
The exit from the new space was not clear of obstructions. The following was observed in the outdoor exit space: old fireplace with chimney stack, a fire pit, chairs, pieces of metal, wood, metal grill/smoker, an old counter top.

The Educator has not held a practice evacuation drill since October. The educator stated that she has not completed a practice evacuation drill with her new full time assistant.

The licensor observed the (REDACTED) to be asleep in the pack and play. The infant was napping with a bib around his neck. This is the Educator’s first safe sleep citation.

The Educator was observed to be diapering on a mat without a second surface to prevent the child from coming into contact with the changing surface.
The Educator used baby wipes to clean the diaper changing surface after diapering a child today.
Exit Safety
The educator has been using an unapproved assistant to help care for the children since September 9, 2025. The assistant’s certificate was approved on September 29,2025,
The Educator’s daily attendance has not been recorded accurately. There is one enrolled child who has not been on the attendance since around June of this year. Additionally the educator has not completed her attendance since 9/24/2025. The educator is not consistently documenting the attendance of her assistants.

A review of the Educator's attendance showed the attendance record for the day of the visit was not accurate. There were 5 children signed in on the attendance and 7 children present. The Educator initially stated there were 8 children present in the child care at the time of the visit.
Judgement
During the visit a (REDACTED) went into the playroom and was playing in there without adult supervision. The licensor brought this to the Educator's attention and she sent her assistant in there to supervise him.

The licensor observed 2 cabinets in the kitchen that were not locked there was Cascade and a pizza cutter accessible. In the bathroom there was air freshener and , a cord for an electric toothbrush accessible on the sink. In the living room there were 2 cords hanging down that need to be secure to the wall

In the kitchen several drawers were not locked and contained hazards such as sharp objects and lighters. There were 2 cords plugged in the living room that were accessible to the children in care. The vacuum was accessible in the living room

The licensor observed a walking treadmill on the floor of the kitchen accessible to the children and one drawer in the kitchen was not locked and contained scissors and sharp knives.

The Educator moved her mother into the home and she has not completed the Change of Household composition transaction and her mother has not completed the required background check.
The licensor observed that the educator did not have the children in care wash their hands prior to giving them a snack.
There is one child in care with an identified allergy on the doctors physical form and it was not posted.
The Educator has been consistently over ratio with the number of children she can care for without an assistant and with the number of children under the age of two, without an assistant since May 7, 2025. The educator has cared for as many as 8 children, none of them school age, without an assistant and up to 5 children under the age of two without an assistant. This occurred from May 7- September 29, 2025. The Educator was cited for this in November 2024.
The licensor observed the following hazards: in the bathroom the cabinet under the sink was not locked and contained shaving cream, face wash and plastic bags and the bathroom trash did not have a cover. The cabinet under the kitchen sink was not locked at the time of the visit and cleaning supplies were accessible.
The pathway on the porch for the second exit was not clear today, there was debris such as a drill, plastic bottles and plant pots that were on the porch and left only a small path clear.
When the licensor arrived for the visit the Educator was the only caregiver present with 7 children, all the children were ages 2 and younger. The Educator was alone with the 7 children for approximately 1 hour.
