Reviewed children’s records. Three files were missing physical description, admission date, and age at time of admission on the first page of enrollment packets. One file needs the annual update. One file needs the general permission to state where child is taken when off premises.
Educator needs to have a seatbelt cutter in first aid kit because she transports children on occasion. Two emergency cards need an updated signature date.

Childrens Records - R Apr 2025
Children’s files were reviewed. There were files that needed the annual update and files that were missing parent signature and dates. One file needs an updated physical. There was a file missing the physical description, date of admission, and age of admission on the first page of the enrollment packet. Reviewed with the educator when she has parents complete the annual reviews for enrollment packets she must ensure there are no sections left blank and all sections have signature and date of signature.

Progress Reports
It was observed that documentation for fire drill for November 2023 and December 2023 were not documented. Educator indicated November drill was just not documented but was conducted and December’s drill was not yet conducted. Licensor had educator conduct fire drill during today’s visit. Educator must ensure monthly fire drills are being conducted and documented.
Emergency preparedness plan could not be reviewed, educator did not have a copy of her parent handbook accessible. Educator was advised to upload a copy of her parent handbook on her document library in her LEAD portal.
Educator indicated that current progress reports were saved in her computer. The computer was not within the daycare space and therefore, not accessible. Discussed with the educator that if electronic files are kept, these must be able to be easily accessible within the approved space for review.
Refrigerator Temperature
Educator’s parent handbook was reviewed. Parent handbook is missing contact information for regional office, does not indicate information regarding lead in home, and child guidance policies are not included. Discussed sick policy, educator is still implementing Covid specific policy this must be included in her parent handbook.

Six children’s files were reviewed. One file was observed to have the name of the previous daycare educator on the enrollment forms. One file did not have the child’s annual update form.

CPR expires on 12/2020 First Aid expires on 12/2021 However, it was observed that the trainer issued the card with a date of 12/16/2019 and an expiration date of 12/21/2019. This is clearly an error that the educator must address with the trainer. Once new card with correct dates are issued, educator will need to submit a picture.
There was no thermometer in the fridge
Toothbrushes were not labeled with date received, this helps ensure toothbrushes are replaced every 3 months
It was observed that the gate leading to deck stairs was not closed.
