During the visit, the following was observed in the Infant/Toddler classroom: an educator changed 2 children's diapers and did not have the children wash their hands with soap and water after the diaper change. A sippy cup and children's plastic plate were observed in the handwashing sink. Per the Licensee, educators are to place items that need to be washed in a red container to be taken. to the kitchen for washing.
Reviewed 2 staff files and observed 1 staff to have completed the incorrect EEC Essentials 2.0 and Refresher course package. All staff must completed the required EEC Essentials 2.0 for the program type they are working within.

2 staff identified on the staff records checklist have expired First Aid. All educators must obtain within six months of employment, and must maintain thereafter current certification of training in basic first aid.
Infection Control/Hand Washing
At the time of the visit, 11 children were observed in the Infant/Toddler Classroom. The maximum group size is 9 children with 2 educators. The program is licensed for 9 Infant/Toddlers. At no time shall a program admit, supervise, or provide care for more than the maximum number of children indicated on the license.
A large fan was observed on the floor in the Infant/Toddler classroom and accessible to the children. In the Preschool 2 classroom, dried food and crumbs were observed at the bottom of the interior of the refrigerator.

During the visit, two Little Tykes plastic slides were not in an area that had an adequate depth of impact absorbing material.
In the Infant/Toddler classroom, plastic bags and ziploc baggies were observed in drawers in the kitchen area. These items were accessible to the children.

During the visit, two staff files were reviewed. One file did not include evidence of 2 references. One file did not include a current PQ Registry.
In the Preschool 2 Classroom, there were 16 children observed in the classroom but only 15 children signed into the classroom.
In the Toddler Classroom, the following was observed: an electric fan was in use and accessible to the children. One bookshelf was not secured to prevent it from tipping.

During the visit, the following hazards were observed outdoors: A Little Tykes climbing structure, three spring horses, and a dome climber did not include an adequate depth of impact absorbing material under and around the fall zones. A railroad tie in the toddler play area is rotting and splintering.

During the visit, four staff files were reviewed. Three files did not include documentation of a current physical (This was previously cited on 4/10/23). Four files did not include evidenced of immunity to Measles, Mumps and Rubella (This was previously cited on 4/10/23). One file did not include documentation of the EEC 12 Essentials Training. Staff observations were not conducted bi-monthly as required.
During the visit, a child was observed sitting on the roof of a Little Tykes Playhouse. Staff did not seem to be aware of this. The licensor asked a staff member to assist the child down from on top of the playhouse.
Child Guidance & Interactions
The rest mats that had been laid out in the Preschool 1 class were approximately four inches away from one another rather than a minimum of two feet as is required by EEC regulations. -In the Preschool 2 room, there were rest mats that had been laid out blocking the egress door to the adjacent classroom. This is a potential safety issue.
During a walk through of the playground, the Licensor observed a toddler aged child had their leg stuck in an entrapment at the bottom of the fence that separates the toddler playground from the preschool playground. No educator seemed to be aware of this, despite the child yelling and crying. The Licensor intervened to have an educator assist the child. Educators must appropriately supervise the children and must be in close proximity to intervene quickly as may be necessary to ensure safety.

During the visit, the Licensor observed a toddler child try to climb under the fence and got their foot and leg stuck in the fence itself. The child was crying and no educator seemed to be aware until the Licensor brought it to their attention.
The program did not have a current Staff Schedule posted in the program.

Upon a review of medication stored at the program, a child's prescribed (REDACTED) was not stored in its original container with affixed prescription label.
Medication Administration Policy
An enrolled child who is prescribed an (REDACTED) did not have a current Medication Consent Form, as the Form was not signed by the child's parent(s). This means in the event of an emergency the program did not have authorization to administer the child's prescribed(REDACTED) in the event of an emergency.
The Staff Schedule was not current and needs to be updated.
Toxic Substances
During an inspection of the facilities, the following was found: -In the Preschool 1 room there was no evacuation procedure posted at the exit near the child's red play kitchen and sink. -In the Preschool 2 class, there was no evacuation procedure posted on the exit door next to the wall where the the fire extinguisher is.

During an inspection of the facility, there were two phone charger cords longer than six inches plugged in and left on a low kitchen counter that was accessible to children. An Educator moved these to a higher kitchen shelf during the visit.
During the visit, the Director stated that the Fire inspection was conducted mid-March of 2022 and she is awaiting the certificate. The Director stated the Building Inspection was scheduled for 3/31/22.

Out of five Children's Records reviewed, one did not have proof of immunizations or documented religious/medical exemption. In the same Children's Record, the physical form indicated an (REDACTED) but there is no documentation of an Individual Health Care Plan.
In Preschool 1 and 2, the Allergy List must be posted in a manner that protects the privacy of the child.

One child's emergency medication had expired on 5/2023.
Staff did not take the children's emergency medication with them to the playground. Therefore the medication was not immediately available if needed.
In the Toddler Classroom, there was no barrier between the children’s activity space and the kitchen.

The Director was not onsite during the visit. When Licensor asked three individual educators who the designee in charge was, no educator was aware of who that person was or where to find requested licensing documents.

A review of three Staff Records at the program indicated the following: -Two educators did not have evidence of a current physical. -Two educators did not have evidence of immunity to MMR.
A walk through of the toddler and playground outdoor space indicated the following: -A rope swing, approximately 10 ft in length was hung from a tree branch with several wooden logs underneath. This was removed by an educator during the visit at Licensor's request. -A rope ladder, approximately 6 ft tall, was hung to a tree for children to climb up. This was removed by an educator during the visit at Licensor's request. -A rainbow hammock was hung between two trees. The program has been cited for this previously. This was removed by an educator during the visit at Licensor's request. -A green and brown hammock was hung between two trees and had several wooden logs underneath. During the exit interview, the Director stated this is used as a "reading area" and the hammock was being used for shade. The Licensor has informed the Director this hammock needs to be removed, as children can easily climb on a wooden log to go onto hammock and this is a potential safety issue. -The fence separating the toddler playground from the preschool playground is not secure on the bottom, causing an entrapment issue. During the visit, the Licensor observed a toddler child try to climb under the fence and got their foot and leg stuck in the fence itself. The child was crying and no educator seemed to be aware until the Licensor brought it to their attention. -Two large rabbits were outside in a fenced in area that was separate from the playground. The program did not have any documentation about the rabbits indicating they are are free from disease and parasites and are licensed and/or vaccinated as prescribed by law.
Out of five Children's Records reviewed, two were lacking a current Individual Health Care Plan. A third Children's Record included a documented (REDACTED) on the child's annual physical. However, the program did not have any documentation of an Individual Health Care Plan for the child.
Upon a review of medication stored at the program, one child did not have a (REDACTED) . This means that in the event of an emergency, a child would not have access to a potentially life saving medication.

A walk through of the Program indicated: -Several windows throughout the program did not have screens. -There was no barrier between the childrens activity space and the kitchen in the Infant/Toddler classroom.
A review of the medication stored at the program indicated: -An over-the-counter medication was not contained in its original packaging. Medications must be in the original manufacturers packaging.
A review of four staff records indicated: -One was lacking evidence of a current physical examination. -One was lacking a current Staff Information form. -One was lacking a complete Staff Orientation form. -Three were lacking two required references. -Four were lacking evidence of bi-monthly observations or annual evaluations.
Upon review of the First Aid Kit stored at the Program, the kit did not contain a cold pack, tweezers, thermometer, scissors, or CPR mouth guard.
A review of medication stored at the program indicated that a prescribed (REDACTED) for a child had expired on (REDACTED).
A review of the Program's IPM plan indicated that the plan was not current.
A review of three children's records indicated: -None contained evidence of current written progress reports as required. Written progress reports must be completed every three months for infants/diverse learners and every six months for toddlers and preschool aged children.
A review of three children’s records indicated: -One lacked a current and complete IHCP. The IHCP plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the childs health if the treatment is not administered.
A walk through of the program indicated that there was no staff schedule posted.
A review of three children's records indicated: -Two medications were listed on the same Medication Consent Form for a child. A Medication Consent Form is required for each medication, outlining the dosage of the medication, the date and time to be administered, possible side effects, and directions for storage.
