General: Based on review of documentation, there was no written schedule of staff hours presented at the time of inspection.
General: Based on observation, the following was found during inspection: 1) A staff (DOH: 08/14/25) had TB documentation on file at the facility from where he had the assessment; however, the document did not have the results marked. 2) A staff (DOH: 09/03/24) had TB documentation on file at the facility from where she had the assessment; however, the document did not have the results marked. 3) A staff (DOH: 08/14/25) did not have documentation of a negative TB test or a statement from a health care professional stating that the staff is free from active tuberculosis on file at the facility at the time of inspection.
General: Based on observation, the following was found during inspection: 1) The library had a stained ceiling tile on the day of inspection. 2) Classroom 139 contained multiple stained ceiling tiles. Classroom 139 had previously been cited for seven (7) stained ceiling tiles. The previous plan of correction stated the school was notified about ceiling tiles, and a work order was submitted to have these replaced. The plan of correction further stated, school maintenance replaced these ceiling tiles on 09/22/25.

General: Based on observation, the following was found: 1) The first-grade group had children ages six (6) and seven (7) with a total of twenty (20) children and one (1) staff. The ratio for children this age is one (1) adult to fifteen (15) children making this group over ratio by five (5) children for at least twenty (20) minutes until another staff arrived. 2) The second-grade group had twenty- six (26) children present with one (1) staff making the group over ratio by one (1) child for at least thirty (30) minutes while the surveyor was at the school. General: Based on interview and review of documentation, the facility did not have a KARES background check list under the license. The staff person in charge indicated difficulties making individual lists under each license since the same email is used for the company.
