General: Based on observation and review of documentation, a lesson plan was not posted in the Four-Year-Old classroom. The staff member in charge of this classroom stated that she keeps her lesson plans in a planner, however the planner was blank for the current week. Additionally, a lesson plan was not posted within the Three-to-Four-Year-Old classroom (107), the staff member had the curriculum written in a daily planner. Animals In Compliance

General: Based on observation, the water temperature of the sink located in the Four's Room was tested with a calibrated thermometer. It was found that the water temperature did not reach above 70 degrees Fahrenheit after allowing the water to run for a few minutes. The water temperature should be no less than 90 degrees Fahrenheit and not greater than 120 degrees Fahrenheit. General: Based on interview and review of documentation, out of thirteen staff files presented for review there were two staff hired 9/9/2020 who did not have documentation of having received training in CPR and First Aid. The staff person in charge stated both staff received training from an individual who is trained to teach CPR and First Aid classes. ECE-TRIS was also reviewed to determine if there was documentation to verify both staff had received training in CPR and First Aid. There was no documentation found in ECE- TRIS.
General: Based on observation and interview, there was a large fenced area located behind the facility. A portion of the area was covered in grass and the other portion had a border made of wood. The interior of the border was filled with mulch and dirt. Numerous holes, pieces of concrete and large metal poles were observed in the ground in a vertical position. A banner of yellow and black caution tape was tied to each side of the fence separating the space with grass and the other space. A blue and green water table was observed in the grass covered area. The staff person in charge stated that playground equipment had been removed from the dirt and mulch covered area several months ago. The staff person in charge stated they are waiting on new equipment to be installed. Further interview with the staff person in charge found that when the children are playing in the grass covered area, staff assemble themselves in front of the caution tape to assure the children do not cross over into the unsafe space. Caution tape/staff is not an acceptable barrier to assure children do not have access to the unsafe area. General: Based on Review of Documentation, a staff member hired on 9/9/15 completed 9.50 hours of cabinet-approved early care and education training during her completed training year. The ECE-TRIS database was also reviewed and conformed these training hours. In accordance with KRS 199.896 (15) and (16), a staff person with supervisory authority over a child shall complete fifteen hours of cabinet-approved early care and education training during each year of employment. 345 - Driver Requirements Not Applicable 922 KAR 2:120. Section 12. Transportation. (18) A driver of a vehicle transporting a child for a center shall: (a) Be at least twenty-one (21) years old; (b) Complete: 1. The background checks as described in 922 KAR 2:110; and 2. An annual check of the: a. Kentucky driver history records in accordance with KRS 186.018; or b. Driver history records through the state transportation agency that issued the driver’s license; (c) Hold a current driver’s license which has not been suspended or revoked during the last five (5) years; and (d) Not caused an accident which resulted in the death of a person. General: Based on Review of Documentation, a staff member hired on 9/9/15 did not have an out-of-state criminal background check on file. The staff member indicated in interview that she lived in Tennessee two (2) years ago. The staff member was not working alone with children during the relicensure survey.
General: Based on Review of Documentation, in room 106, a two (2) year-old room, there were no activities listed on the posted lesson plan. There were categories listed on the plan, but no activities listed in each category. The classroom teacher stated that she completes the lesson plan as the day goes on and it is on a dry erase sheet.
General: Based on Review of Documentation, it was found that an employee hired 08/07/2014 did not complete orientation training until 01/28/2015. 345 - Driver Requirements Not Applicable 922 KAR 2:120. General: Based on Review of Documentation, 1. Staff person hired 8/14/08 did not have evidence of negative T.B. results on file in the facility. 2. Staff person hired 1/1/13 did not have evidence of negative T.B. results on file in the facility prior to employment. The results of the staff person's results were dated 1/23/13.
General: Based on Review of Documentation, 1. A volunteer was observed leading an activity with the children in the Fours classroom. A review of files found that the volunteer did not have the results of or evidence of the application for a criminal background check or a CA/N check. 2. Staff person hired 1/1/13 had the results of a criminal records check dated 1/23/13. There was no evidence on file that the application for the check had been submitted prior to employment as required. 3. Staff person hired 1/1/13 had the results of a CA/N check dated 1/22/13. There was no evidence on file that the application for the check had been submitted within the first five (5) days of employment as required.

General: Based on interview and review of documentation, a staff member hired on 8/6/2019, did not complete the required PAHT training. This staff member has been employed for more than one year. The ECE-TRIS system was reviewed. The staff person in charge indicated the staff person has taken the required PAHT training. A review of this same staff person's file and ECE-TRIS records found this staff person only received 14.75 hours of annual training. The staff person in charge indicated the staff person completed 15 hours of annual training. The staff person stated further she e-mailed ECE-TRIS twice on 8/24/2021 regarding .25 hours of training that has not been credited to this staff person's training hours. General: Based on interview and review of documentation, it was found the children enrolled in the Pre-K Room are in attendance from 9:00 a.m. - 3:00 p.m. A review of the daily schedule posted in the classroom found that the children enrolled in this room have Morning Movement from 9:30 a.m.- 9:45 a.m. and Large Motor Activities from 1:55 p.m.- 2:25 p.m It should be noted that bathroom time and handwashing time were documented to take palce at this time as well. This only allows the children 45 minutes of exercise/outdoor play during a full day of care. Premises Not In Compliance
General: Based on observation, the following was found: 1) In the Two's Room there was an area of paint that had peeled away from the surface on the wall near the TV. 2) In the Four's Room a line of black scuff marks was found on the wall behind the trashcan.
General: Based on review of documentation, found the following: 1) The time of day a monthly fire drill was conducted in December of 2020 and March of 2021 was not documented. 2) The time of day a quarterly practiced tornado drill and a quarterly practiced earthquake drill conducted in March of 2021 was not documented. General: Based on Observation, the parents pack lunches for the children enrolled in the child care facility. The facility supplements milk to the children if it is not packed in their lunch. The milk in the refrigerator was 2 percent milk. The director stated that she was unaware that the regulations required a certain percent of milk to be served to the children. Pasteurized low fat one (1) percent or fat-free skim milk shall be provided for children ages twenty-four (24) months to school-age. 990 - Bottle Preparation by Parent Not Applicable 922 KAR 2:120. Section 9. Food and Meal Requirements. (4) Formula or breast milk provided by the parent shall be prepared and labeled.
