General: Based on review of documentation, the following was found: 1) A staff (DOH: 08/01/24) had TB results on file at the facility; however, they were dated 08/14/24 which was after the staff was hired. 2) A staff (DOH: 08/01/24) had TB results on file at the facility; however, they were dated 08/21/24 which was after the staff was hired. 3) A staff (DOH: 10/13/25) did not have documentation of a negative TB test or a statement from a health care professional stating that the staff is free from active tuberculosis on file at the facility at the time of inspection.

General: Based on review of documentation, the following was found: 1.) Staff member (DOH: 08/10/23) completed nine (9) out of fifteen (15) hours of Cabinet- approved early care and education training for the 08/10/23--08/9/24 training year. 2.) General: Based on observation, this regulation was not met. There was an electrical outlet in the Toucan classroom that did not have a face plate big enough and the metal casing was exposed. Also, there were chairs stacked in the Train classroom that created a tipping hazard. General: Based on review of documentation, the Statement of Deficiency and corresponding Plan of Correction from the 2023 licensure year was not posted for public inspection. The most current plan posted was dated 2022. Animals Not Applicable
General: Based on observation, the following was found: Outdoor Play Area: 1.) Two (2) toddler-sized slides/ climbers on the playground did not have protective surfacing underneath. Both climbers were placed upon grass. 2.) One (1) toddler-sized slide/climber was placed against a brick wall on the outdoor play area.
