General: Based on review of documentation, the following was found: 1) A staff (DOH: 08/01/24) had TB results on file at the facility; however, they were dated 08/14/24 which was after the staff was hired. 2) A staff (DOH: 08/01/24) had TB results on file at the facility; however, they were dated 08/21/24 which was after the staff was hired. 3) A staff (DOH: 10/13/25) did not have documentation of a negative TB test or a statement from a health care professional stating that the staff is free from active tuberculosis on file at the facility at the time of inspection.

General: Based on observation, the following was found: Outdoor Play Area: 1.) Two (2) toddler-sized slides/ climbers on the playground did not have protective surfacing underneath. Both climbers were placed upon grass. 2.) One (1) toddler-sized slide/climber was placed against a brick wall on the outdoor play area.
