One group had 12 students, and the other had 13 students for a total of 25 students. The ratio for this group is one adult to 15 children. 2.) The third grade and fifth grade groups were being supervised by one staff, leaving these groups out of ratio for approximately 25 minutes. General: Based on Interview, it was found that at on 4-12-18 at 3:16 p.m., a group of children finished snack and was taken to the bathroom prior to going outside to play. Staff reported that female staff are not allowed in the boys' bathrooms, so another child was sent to make sure everyone had exited. A headcount was not completed prior to leaving the area and taking the children outside to play at 3:20 p.m. A six-year-old child was left alone in the bathroom at this time. While the group was outside, the child left the bathroom, passed behind the staff person sitting at the check-out table in the lobby, walked approximately thirty-five (35) feet to the Primary Hallway, down the Primary Hallway (which is approximatley 333 feet long) and out a side door which opens onto a grassy area beside the school. The teachers in his group did not realize the child was missing until a school teacher brought him back inside the building at approximatmely 3:26 pm.
