General: Based on observation, several crib matress sheets in the Infant Room were loose and not tight fighting.
General: Based on observation, the following was found: Toddler Two Room 1) The racetrack rug had fraying yarn around the sides. 2) The mermaid plush toy near the reading center was stained. 3) The shelf near the art center was bulging in the back causing the particle board to be exposed. Preschool Three Room 1) The large rug with squares and numbers was stained. 2) The blue and white soft chair in the reading center was stained on multiple sides.
General: Based on review of documentation, one (1) staff (DOH: 08/15/23) did not have current documentation of a negative TB test or a statement from a healthcare professional stating staff is free from active tuberculosis on file at the facility. The documentation on file was dated 08/02/23.
General: Based on interview, on 08/04/25 after nap time, it was found that a three (3) year-old child went to use the bathroom alone in one (1) of two (2) stalls and the teacher did not have him in the range of sight as she was helping another child in the second stall. The three-year-old child got hurt as the toilet lid fell on his penis while he was unsupervised. Staffing Requirements In Compliance
General: Based on interview and review of documentation, on 08/04/25 after nap time, a three (3) year-old child went into one of the stalls to use the bathroom alone. This child uses the toilet standing, and the toilet was too high with no accommodations. The child had to stand on his toes and put his penis on the toilet rim to use the bathroom. The toilet lid fell onto his penis and the child sustained a bruise on tip of his penis and there was bloody discharge. The teacher did not witness this incident as she was putting another child on the toilet in the other stall but after the injury the first aid was applied to the injured child and parent was notified via Bright Wheel app.

A staff member (DOH: 06/08/23) completed only 7.0 hours of the required fifteen (15) hours of early care and education training during the training year 07/01/23 to 06/30/24. 5. A staff member (DOH: 06/08/23) completed only 7.0 hours of the required fifteen (15) hours of early care and education training during the training year 07/01/23 to 06/30/24. 6. A staff member (DOH: 02/12/24) has not completed orientation training, which is required within ninety (90) days of employment. General: Based on observation, the following was found: Toddler 1 Room 1. One (1) uncovered electrical outlet was located on the lower section of the wall near where the cots were stacked. Gym 1. A small gross motor area of the gym was sectioned off and a group of toddlers were observed playing in the area during the renewal survey. This area contained a tall wooden staircase that leads up to a storage loft. The stairs to the loft were steep and there was no barrier to prevent the toddlers from climbing the stairs to the loft area. The loft contained multiple boxes, a T-shirt press, and stacked materials. 2. An unsecured large piece of plywood was leaning against two (2) walls to close an opening in the back of the room.
