General: Based on interview and observation, the childcare center does not keep children’s personal information including name, address, phone number, and pick-up list when the children are being transported in the childcare center vehicle.

Based on observation and interview, the director failed to notify the Division of Regulated Child Care of a change in operating hours. The surveyor attempted to conduct re-licensure inspections on 11/28/18 at approximately 9:44 a.m. and on 12/10/18 at approximately 9:23 a.m. but found the type II child-care center closed. The director later notified the surveyor to say that she only cares for school-age children. She stated she leaves her home at approximately 2:00 p.m. to pick up children from public schools and arrives home at approximately 3:30 p.m. She stated that she operates from 7:00 a.m. until 6:30 p.m. when school is not in session. General: Based on Interview, it was found through interview with the staff in charge that when the driver runs to pick up children for the facility, no one is present at the facility with CPR/First Aid. The staff in charge stated that her CPR/First Aid expired on 08/26/2017 and stated that the driver is the only individual that currently has CPR/First Aid.
General: Based on Observation, it was found that there were four (4) children present ranging from infant to two (2) years of age that were all being cared for together in the same space. In an interview with the staff in charge, she stated that she always keeps the children combined together. Premises Not In Compliance

General: Based on Observation, a trampoline located in the middle of the outdoor play area, was accessible to the children in care. The netting around the trampoline contained an opening that was unzipped and unlatched. The director indicated that she does not allow the daycare children to use the trampoline. In addition two (2) wooden planks of the privacy fence contained sharp, rusted, nails that were exposed. These nails were within reach of a child. Several large tree limbs were observed on the ground inside the outdoor play area. A gap was observed in the wooden privacy fence where it meets the side of the house. Two (2) wooden boards were placed leaning into the gap, but these boards were not secured. General: Based on Review of Documentation, three (3) staff members did not complete fifteen (15) hours of cabinet-approved early care and education training during their completed training year. One (1) staff completed one (1) hour of training and the other two (2) staff did not complete any hours. All three (3) staff were hired on 2/4/11. The ECE- TRIS record and the staff files were reviewed. In addition, one (1) staff did not completed the 1.5 hours of pediatic abusive head trauma training. This staff was hired on 2/4/11. General: Based on Review of Documentation and interview, this regulatory requirement was not met. A review of transportation documentation on file for two (2) children enrolled at the facility during June 2016 revealed that the facility did not have a written alternate plan in place to use if the designee was not available to receive the child from the center. The director/owner stated during interview that she had a verbal agreement with the mother that it was acceptable to drop off the two (2) children at home with an older sibling (twelve (12) year old) if the mother was not home; however, nothing in writing.
General: Based on Review of Documentation and interview, this regulatory requirement was not met. During a complaint investigation, the surveyor requested to review the faclity's transportation documentation for June 2016. The director/owner stated that this documentation had been misplaced. The director/owner stated that transportation routes for the daily trips to and from the home of two (2) children enrolled at the facility (seven (7) year old and four (4) year old) and transportation routes for numerous field trips identified on parental permission slips were included in this documentation; therefore, not available for the surveyor to review. Furthermore, a review of the binder that is kept on the vehicle found that the last documented transportation emergency drill was 12/2015; therefore, these drills are not conducted monthly.
General: Based on Review of Documentation and interview, the facility failed to report an injury to a child that required medical care to the Cabinet within twenty-four (24) hours of the time of discovery. Interviews and record review during the investigation revealed that on 6/13/16 at approximatley 2:30PM children were playing with magnetic blocks on the floor when a seven (7) year old child was accidentally hit in the eye by another child. The director/owner stated during interview that she asked the child if she was okay after the incident and looked at the child's eye to see if there was an injury. The director/owner stated, "The child rubbed her eye a couple times, but told me that she was all right. She was with me a couple more hours before I took her home. Her eye never appeared red or irritated, so I didn't fill out an incident report or tell her mom about it". The child's mother stated during interview that while at home later that evening the child informed her of the incident and stated that her eye was hurting. The mother stated that she looked at the child's eye and did not observe any signs of injury; however, encouraged the child to lay down for awhile in an attempt to make her feel better. The mother stated that at approximately 8:00PM that evening the child came to her complaining that her eye was still hurting and at this time the child was having difficulty opening her eye because of the pain. The mother stated that she contacted the director/owner via phone call at this time to find out details about the incident and then transported the child to the emergency room for treatment. Documentation from the emergency room visit states that the child was diagnosed with a corneal abrasion. The child was sent home with Erythromycin Ophthalmic eye ointment to use on her injury. The mother and the director/owner stated that the child returned to the facility the next day (6/14/16). The mother and the director/owner stated that they did not have a conversation about the emergency room visit; however, both stated during interview that the child informed the director/owner that she went to the emergency room for treatment on her eye. This incident was not reported to the DRCC office by the director/owner. Director Requirements

General: Based on Observation, DRCC surveyor attempted a relicensure visit on 10/16/13 at 9:00am and was told by the staff person in charge that she was about to leave to take her daughter to her college class. She stated that she was going to take the daycare children with her on the van while she took her daughter to class and then she was going to pick up more daycare children before returning back to her home sometime after 10:00am. The daycare children would be sitting on the van for over an hour while she completed her stops. She stated that she takes her daughter to college at least a couple of days a week and takes the daycare children along with her each time. The children do not have any programmed activity during this period of time away from the home.
