General: Based on observation and interview, at approximately 2:40 pm on 1/31/2024 the surveyor stood outside of the gym in the hallway and observed three (3) children leave the gym and enter the bathrooms located beside the gym. The children were not supervised by a staff member. There were four (4) staff inside of the gym at the time. None of the staff were in scope of vision or range of voice of the three (3) children who left the gym. An interview with staff found that staff are supposed to stand outside of the gym doors when children leave to go to the bathroom. The staff member also stated that only one (1) child is allowed to leave at a time. However, at the time of the follow-up inspection the surveyor did not observe any staff member standing outside of the gym doors when the three (3) children left the gym. It should be noted that the children were observed returning from the bathroom and re-enter the gym. The age of the children in the gym at the time were ages five-to-eight years old.
