General: Based on review of documentation, there was no written course of action for shelter-in-place posted with the diagram for emergencies.
General: Based on review of documentation, three (3) staff (DOH: 10/13/04, 09/15/23, and 08/29/23) did not have evidence of a current evaluation at the time of inspection.
General: Based on review of documentation, the following was found during inspection: 1) A staff (DOH: 10/09/25) did not have up to date documentation of a negative TB results or a statement from a health care professional stating that the staff is free from active tuberculosis on file at the facility. The document on file expired on 03/12/26. 2) A staff (DOH: 06/25/25) did not have documentation of negative TB results or a statement from a health care professional stating that the staff is free from active tuberculosis prior to employment on file at the facility. 3) A staff (DOH: 07/18/25) had TB documentation on file at the facility from where she had the test placed; however, the document did not have the results of the test.
General: Based on observation, the refrigerator in the Sunfish Classroom did not have a thermometer so the temperature could not be determined.

General: Based on interview and review of documentation, During the interview process it was determined that the child was being allowed to play in the high chair by climbing in and out of it, and this action resulted in her falling face first out of it on the floor injuring her mouth. General: Based on observation, in the Minnows Classroom, one (1) soiled crib sheet was found. It should be noted the child that crib was assigned to was in an activity center for the duration of the time the surveyor was in the room.
General: Based on observation, the following was found: Sailboats 1. The outlet in the area of the number rug was uncovered. 2. Twenty-two (22) cots were stacked against a wall and were observed to be wobbly, posing an injury hazard. 3. A damp mop was observed at the same height and directly to the left of the handwashing sink for the children in care.
General: Based on review of documentation, one (1) child in care (DOE: 04/16/24) had an enrollment file containing an expired immunization certificate. General: Based on review of documentation, two (2) staff (DOH: 10/06/25 and 05/25/25) did not have evidence of completing the required CPR/First Aid training on file at the facility
General: Based on review of documentation, the following was found during inspection: 1) A staff (DOH: 10/13/04) completed eight (8) of their fifteen (15) hours of annual training for the year dated 07/01/24 to 06/30/25. 2) A staff (DOH: 09/15/23) completed eleven (11) of their fifteen (15) hours of annual training for the year dated 09/16/24 to 06/30/25. 3) A staff (DOH: 08/29/23) completed nine (9) of their fifteen (15) hours of annual training for the year dated 08/30/24 to 06/30/25. 4) A staff (DOH: 10/09/25) completed pediatric abusive head trauma training on 01/09/20; however, did not renew the training due 01/09/2025. 5) Five (5) staff (DOH: 07/18/25, 06/25/25, 09/30/25, 10/06/25, and 05/20/25) did not complete the required cabinet-approved orientation training and has been employed for more than three (3) months. General: Based on observation and interview, there was no evidence at the time of inspection of a written policy to maintain supervision during breaks and movement from the classroom. General Administration Not In Compliance
General: Based on observation, the cots in the Tugboats Classroom were standing up on their ends in the corner without a barrier to prevent them from falling over on the children and posed a safety hazard.
General: Based on observation, the vent in the Sailboats bathroom had a build up of debris preventing proper air flow.
General: Based on observation, the Tugboats Classroom had a ceiling light that was not working and one (1) that was missing the cover on the day of inspection.
General: Based on observation, the refrigerator in the Infant Classroom had a freezer that was frozen over and in need of defrosting.
General: Based on observation, there were three (3) bottles in the refrigerator in the Sunfish Classroom without lids. Food Service In Compliance
General: Based on review of documentation, three (3) staff (DOH: 10/13/04, 08/13/19, and 08/29/23) did not have a professional development plan on file at the time of inspection.
General: Based on interview and review of documentation, there was no evidence at the time of survey of a written allergy care plan of treatment in the event of an allergic reaction for children in care with allergies to milk or raspberries.
General: Based on observation, the following was found: Sunfish 1. Two (2) ride-on toys available to children in care had an accumulation of dirt and grime. Dolphins 1. The rug covered with various sized squares had an unidentified hard substance stuck to the fibers.
General: Based on observation and interview, the following was found: Older Children's Playground 1. A long, rectangular, red tabletop was observed beneath a swing. A staff member stated it was there to cover a puddle from recent rain to allow children in care to still utilize the swing. 2. Several large roots were exposed beneath the grey and blue swingset.
General: Based on observation, the Sunfish Classroom had a bookshelf available, but no books were seen in the room.
General: Based on review of documentation, one (1) staff member (DOH: 01/15/25) had a TB skin test on file that had expired 04/05/25.
General: Based on observation, one (1) ceiling lightbulb cover was missing in the Tugboats Classroom.
General: Based on observation, the wall of the reading nook in the Tugboats Classroom had red crayon marks.
General: Based on observation, in both the Starfish Classroom and the Dolphins Classroom, the toilet used for children in care had waste that had not been flushed. Hygienic Practices Not In Compliance
General: Based on observation, the trash can used for diapering in the Starfish Classroom did not have a cover. It should be noted the lid was observed on the bottom shelf of the changing table.
General: Based on observation, the following was found: Older Children's Playground 1. The protective surface had worn away to reveal the ground beneath the black swings. 2. The protective surface had worn away to reveal the ground beneath the blue swings.
General: Based on review of documentation, the following was found: 1. One (1) child's enrollment file (DOE: 03/30/25) did not contain contact information for their preferred hospital. 2.Two (2) children's enrollment files (DOE: 08/12/22 and 09/03/24) did not contain a preferred physician or contact information for that preferred physician.
