922 KAR 2:090. Section 9. Records.
General: Based on interview and review of documentation, the following was found: 1. Two (2) children's files (DOE: 03/06/26 for both) had immunization certificates available for review that had expired as of 03/18/26. The staff in charge indicated the parent was waiting on their one-year-old appointment to update their immunization records. 2. A child's file (DOE: 03/10/26) contained a print-out of immunizations completed from myChart. This record did not contain a date until which the immunizations would be considered current nor the signature of a medical professional.
