General: Based on observation and interview, there was a ceiling light that was not working in the Toddler E Classroom on the day of inspection. General: Based on observation, the entryway to the Preschool Classroom had areas of chipped paint on the wall on the day of inspection.
General: Based on observation, chairs were stacked on top of a table in the Toddler C classroom which posed a safety hazard for children.
General: Based on observation, the sink in the Toddler B Classroom was not adjacent to the changing table on the day of inspection. The sink was on the opposite side of the room, requiring staff and children to cross the room to reach the sink to wash hands.
General: Based on review of documentation, the following was found during inspection: 1) Three (3) children files (DOE: 06/24/25, 06/09/25, and 06/06/25) did not have preferred hospital contact number listed. 2) Two (2) children files (DOE: 06/24/25 and 06/09/25) did not have physician contact information on file. General: Based on observation and review of documentation, an employee (DOH 09/11/2023) had a TB skin test printout which stated it was negative, but no name was on the printout. General: Based on observation, a window seal in Infant Room A and B had chipped paint on the surface that was within reach of children. Further observation found another window in the Infant A and B room had a broken lock on the window which had pointed medal protruding near a climbing device/toy that is used by children in care.
