General: Based on observation, interview and review of documentation, the following was found: 1) On 04/14/26, a three-year-old child was left alone in the Preschool Classroom (Room #108) from 8:34 a.m. to 9:04 a.m. A staff member transitioned from the classroom to the cafeteria, but the child did not leave the classroom with the rest of the class. He was discovered in the classroom by a Fayette County Public School employee and taken to the cafeteria. 2) On 04/16/26, the surveyor observed a staff member transition twelve (12) children, ages three to four (3-4) years old from the classroom to the cafeteria utilizing the same route that was used on 04/14/26. During that transition, the children were in a straight line with the staff member at the back of the line. As they reached the end of a hallway within the center, six (6) of the children turned the corner and proceeded to the cafeteria without supervision. Once the staff member entered the cafeteria with the remaining six (6) children, she told the cafeteria staff to have her children sit at tables assigned to her in the middle of the cafeteria, but she did not count the children to ensure they had made it to the cafeteria. General: Based on interview and review of documentation, it was determined that a staff member grabbed a child by their sleeve while on the playground. The staff member attempted to pull the child toward her to direct him not to throw leaves at her. While being pulled the child tripped over the plastic border that the staff was sitting on. General: Based on interview, on 10/25/22 a three-year-old child was left alone without adult supervision in a classroom for approximately twenty (20) minutes. There were a total of six (6) preschool children with three (3) staff present the day of the incident. The three (3) staff took the preschool children out onto the playground, which is accessed from their classroom, at approximately 11:00am; after several minutes outside, the staff realized that one (1) of the children was not on the playground. After looking for the child for several minutes, one (1) of the staff went back to the classroom and found the child on the floor in the block area. The child was found unharmed. General: Based on interview, it was determined that a staff person used profanity in front of a child by spelling out the word in the child's presence. Further, it was found that there have been instances when two staff members did not allow children to play together in centers or eat with other children at lunch time.
General: Based on observation and interview, the child-care facility failed to meet the regulations as follows: 1) The surveyor observed a discolored water stained ceiling tile in the corner of the ceiling located in classroom 105. The staff person told the surveyor that the air-conditioning was replaced recently. She stated she reported the ceiling tile to the staff person in charge. 2) The surveyor observed a hole in the wall located in the restroom adjacent to classroom 105. The surveyor observed a large cement block that was in an upright position, slightly leaning and positioned on top of a tree root on the playground. At the time of the observation at approximately 10:15 a.m., six (6) toddlers and two (2) staff were on the playground. The surveyor noted at approximately 10:18 a.m., two (2) female toddlers went over and touched the cement block. At the time of the survey it was found that none of the employee files were available for review; therefore, there was no information available to determine if the staff present and working with the children had the required background checks (criminal records check and child abuse/neglect check). The staff person in charge stated that the staff files were locked in a file cabinet in an administrative staff person's office and indicated that this staff person was not present because she was on vacation. The staff person in charge identified a total of twenty-six (26) staff that currently work at the facility.
