General: Based on interview and review of documentation, one (1) child's enrollment file (DOE: 04/08/24) contained an immunization certificate that had expired as of 03/01/25. The staff in charge indicated she had not received an updated certificate.
General: Based on interview and review of documentation, a staff member (DOH: 02/26/25) obtained a background status of eligible for hire on 05/13/25, approximately two and one half (2.50) months after their date of hire. The staff in charge indicated there was an issue with fingerprinting that caused a delay; however, the staff member did continue to work during the period between the date of hire and the eligibility date.
General: Based on review of documentation, the following was found: 1. Two (2) children's enrollment files (DOE: 12/04/24 and 08/13/25) did not contain a contact telephone number for their preferred hospital, nor authorization to obtain emergency medical treatment if necessary. 2. Five (5) children's enrollment files (DOE: 08/11/21, 07/01/22, 08/10/22, 08/10/22, and 07/01/24) did not contain a contact telephone number for their preferred hospitals. 3. Three (3) children's enrollment files (DOE: 08/16/23, 04/08/24, and 04/08/24) did not contain a preferred hospital, contact information for said hospital, nor authorization to obtain emergency medical treatment if necessary.

Two (2) staff members (DOH: 09/06/24 and 09/12/25) did not have TB skin tests nor statements from a medical professional stating they were free from active tuberculosis available for review at the time of survey. 2. A staff member (DOH: 07/16/24) had a TB skin test available that had expired on 10/04/25 and had not yet been renewed at the time of survey. 3. A staff member (DOH: 09/11/23) had a TB skin test available that had expired on 10/03/25 and had not yet been renewed at the time of survey.
General: Based on review of documentation, the following was found: 1. Two (2) staff members (DOH: 07/16/24 and 09/06/24) had not completed their six-hour orientation training at the time of survey. These two (2) staff members had been employed more than three (3) months. 2. Two (2) staff members (DOH: 07/16/24 and 09/06/24) had not completed their pediatric abusive head trauma training at the time of survey. These two (2) staff members had been employed more than one (1) year. 3. A staff member (DOH: 07/01/24) completed thirteen (13) of the required fifteen (15) hours of annual training for the year dated 07/01/24 to 07/01/25. 4. A staff member (DOH: 10/02/23) completed eleven (11) of the required fifteen (15) hours of annual training for the year dated 10/03/24 to 06/30/25. A review of ECE-TRIS confirmed these findings.

General: Based on interview, it was confirmed that the facility did not provide for the health, safety, and comfort of a child when the child was pushed from the top of a slide. Interviews with staff indicated that at first no one knew exactly what happened to the child; however, the child told the parent that another student pushed him from the top of the slide, resulting in the child falling and breaking his arm in two places. Although staff was within sight and sound of the children on the playground, staff's attention was directed toward another slide. Thus, a child's safety was jeopardized.

Written documentation received in the DRCC office from the staff person in charge at the facility stated that an eight (8) year old child hit her arm on a fire extinguisher box on 2/25/15 at 4:45PM and fractured her left arm/wrist. The written documentation further indicated that the child's mother picked the child up after the incident to seek medical attention.
