The relevant adult and the provider both confirmed an appointment would be made immediately to obtain the appropriate documentation demonstrating the relevant adult was free from active tuberculosis.
General: Based on review of documentation, the provider (DOH: 07/03/01) had previously taken the pediatric abusive head trauma training on 03/06/20, implying a renewal date on or before 03/06/25. At the time of survey on 03/03/26, the pediatric abusive head trauma training had not been renewed as is required every five (5) years. A review of ECE-TRIS confirmed this finding.

General: Based on observation, while the surveyor was assessing the child care space, the provider was witnessed wiping two (2) children's noses. The provider did not complete proper handwashing after either child's noses were wiped.

Of the six (6) children's enrollment files reviewed, zero (0) contained contact information for their respective preferred hospitals. 2. A child's enrollment file (DOE: 08/01/23) did not contain a contact telephone number for their preferred physician.
General: Based on review of documentation, the provider did not have a current annual plan (professional development plan) to ensure the training hours are completed. The plan on file was dated 2023.

General: Based on Review of Documentation, it was found that during the survey the provider was caring for seven (7) children under the age of six (6)years at one (1) time. The children in care ranged from four (4) months-old up to three (3) years-old.
General: Based on review of documentation, no written evidence of shelter-in-place/lockdown drills was located. The provider indicated that she conducted the drills, but was unable to locate the documentation.
General: Based on observation, the kitchen contained an electrical outlet that was not protected. The bottom part of the outlet contained something that was plugged in and the top part of the outlet was not protected.
General: Based on review of documentation, evidence presented for two (2) staff members (DOH: 07/03/01 and 01/03/16), included the most recent TB skin tests dated 02/03/23 and 02/21/19, which were no longer current.
General: Based on review of documentation, the following was found: 1) The provider (DOH: 07/03/01) did not complete any of the required nine (9) hours of early care and education training during the training year 07/01/23 to 06/30/24. 2) The provider’s assistant (DOH: 01/03/16) completed pediatric abusive head trauma (PAHT) training on 02/15/17; however, did not renew the training due on 02/15/22.
