General: Based on review of documentation, one (1) staff member (DOH: 08/28/24) had not completed the mandatory orientation training as of the surveyor’s visit on 04/17/25. This training is required to be completed within ninety (90) days of hire. General: Based on interview and review of documentation, one (1) staff member (DOH: 08/28/24) did not have an eligible for hire background check status as of the surveyor’s visit on 04/17/25. The surveyor identified the date of hire through interview with the staff member. It should be noted, the Director later told the surveyor, via telephone, that the staff member had been hired 04/03/25, but had not begun working until 04/16/25, and at no point was alone with children in care. The surveyor informed the Director that while she had been present in the facility the morning of 04/17/25, the staff member had been alone with children in care. Furthermore, the surveyor reviewed a photo of an incident involving a child’s pants being damaged by the staff persons usage of cleaning supplies that was dated 02/27/25. The KARES database was reviewed and identified the staff member to have an “in process” determination status as of 04/14/25 and a final determination of eligible on 04/23/25. General: Based on review of documentation, 1.) Staff member (DOH 01/20/20) had two out of fifteen hours of Cabinet- approved early care and education training recorded on ECE-TRIS for the July 1, 2022 – June 30th, 2023, training year. The staff member indicated that they had completed several training courses via “Universal Class”, however, these were not reflected on ECE-TRIS. 2.) Staff member (DOH 03/30/23) completed zero out of fifteen hours of Cabinet- approved early care and education training for the 03/30/23 – 03/29/24 training year. 3.) Staff member (DOH 03/30/23) did not have evidence of completing Cabinet- approved early care and education training. This training is required within the first 90 days of employment. 4.) Staff member (DOH 03/30/23) did not have evidence of competing Pediatric Abusive Head Trauma training, this training is required within the first year of employment. General: Based on review of documentation, the child care center failed to comply with regulatory requirements. A current report from the State Fire Marshal was not presented for review at the time of survey, as the last one on file was dated 04/22/2021. A subsequent interview with the center director found she had not attempted correspondence with the State Fire Marshall to inquire about an inspection.
General: Based on observation, the child care center failed to maintain state regulations. During the survey of the center, the following was found: 1.) Unprotected electrical wall outlets were found in the Pre-K Room, One's Room, and Two's Room. 2.) An electrical outlet, located on the back light blue wall, was loose to the touch and separating from the wall. 3.) Jackets, backpacks, and other personal items belonging to children were found hanging on hooks and freely touching each other in the upstairs Pre-k Room. During a tour of the facility, the surveyor observed an oven turned on in the kitchen and both doors used to enter the kitchen were propped open. There is not a sink in the classroom; therefore, staff and children have to exit the room and enter the bathroom down the hall to access a sink after diaper changes. General: Based on observation, the following was found: 1) There were numerous pieces of paper, trash and dust found in the large floor return vent in the preschool classroom. 2) The quarter round in the two-year-old classroom had numerous areas of chipping paint. General: Based on observation, the following was found: 1) In the preschool classroom, there was an area of pencil marks on the wall in the back left corner of the classroom. In addition, there were areas of missing/chipped paint on the baseboards all around the classroom. Also, there were two holes in the wall underneath where the pineapple art was on the wall. 2.) In the school age classroom, numerous holes and marks were found on the wall. Also, there were areas of missing/chipped paint on the baseboards. 3.) In the infant classroom, the closet door in the first classroom had areas of missing/chipped paint. 4.) In the classroom located upstairs, there were areas of missing/chipped paint, and a hole in the wall to the left of the closet. Also in this classroom, the hardwood floors were very worn and in disrepair.
General: Based on observation, the following was found: 1.) In the downstairs restroom, there was a container of bleach being stored next to the washer and dryer. During the walkthrough of the restroom, the baby gate was not engaged; therefore, this was accessible to the children. 2.) In the storage closet upstairs, there were numeous gallons of paint, as well as toilet bowl cleaner. This closet was unlocked at the time of the inspection and accessible to children. 3.) In the broom closet located on the main level, there were several different cleaning supplies such as bleach, soap, hand sanitizer, and trash bags being stored. This closet was unlocked, leaving all items accessible to the children in care. 4.) In the upstairs classroom, there was a window air conditioning unit that was plugged into the wall. The cord to this unit was hanging down and not secured properly to ensure it was out of reach of the children.
General: Based on observation, this regulatory requirement was not met. An inspection of first aid supplies located on the van used to transport children found that a working flashlight and liquid soap were not available. The cold pack located with the materials appeared to have burst. There was not another cold pack on the vehicle.
General: Based on observation, this regulatory requirement was not met. 1) The cord from a Home Medics brand radio was observed plugged into the wall by a crib in the sleeping area used for infants and toddlers. The cord was loose and accessible to the children. 2) In a laundry room/bathroom area located off of the sleeping area used for infants and toddlers, a toilet brush and toilet plunger were observed on the floor by the toilet. There is not a door or gate to assure children do not enter this area making these items accessible to the children. 3) In the room used for activities for the infants and toddlers, there were five (5) chairs stacked on top of a low table. Children ages one to two (1-2) years old were observed playing in this room at the time of the survey. This is a safety hazard for the children. 4) In the preschool room located on the middle floor of the facility, there was an electrical outlet that was missing a plug cover. The outlet was located beside the blue sensory table making the uncoverd receptacle accessible to the children.
General: Based on observation, this regulatory requirement was not met. Upon arrival at the facilty a tour of the facility was taken. In the infant/toddler play area two (2) children who are one-year-old were observed sleeping in a swing. The children were not placed in a crib or on mat to sleep. In the infant/toddler sleeping area there were three (3) cribs. Each of these cribs had sheets that were loose fitting and did not fit tightly over the mattress.
General: Based on Interview, the director stated that she did not have transportation records documenting the times children are on and off of the van. Therefore, the surveyor was unable to verify the times children are transported to and from the child care center. Food Service/Food Program In Compliance Food Service Not In Compliance
General: Based on Observation, the surveyor noted the window in the bathroom was opened. Although the window was screened, the surveyor noted there were several dead moths, flies and a cobweb in the window sill.
General: Based on Interview, the director stated the public school bus drops off five (5) school-age children at the end of the school day at the facility when school is in session and then those five (5) children are transported by her in a 2001 Windstar mini-van to the owner's other day care facility located on Cambridge Drive which is 0.7 miles away. She stated the school bus does not make a stop at the day care located on Cambridge Drive and that is why she has to transport the children to that location. The director stated she does not complete pre-trip daily inspections of the vehicle prior to using it to transport children. General: Based on Observation and interview, a twenty-three (23) month old child was observed walking around the Toddler classroom while drinking a bottle. The child was not seated for a meal. The Director stated that the child's mother requested that the child have a bottle with her at all times.
General: Based on Review of Documentation, 1. Two (2) children, both enrolled 3/1/16, did not have the name and contact information for a physician or the name of a preferred hospital listed in their file. 2. A child enrolled 3/11/16 did not have the name and contact information for a physician, the name of a preferred hospital listed or permission for emergency medical care signed by the parent on file. The Director was not aware that the children's enrollment forms had not been filled out completely by the parents.
General: Based on Observation, it was found that the youngest child in the Toddler room was thirteen (13) months old and the oldest was twenty-seven (27) months old. In an interview with the Director it was stated that these children are grouped together all day.
General: Based on Observation, the changing mat in the Toddler classroom and the changing pad in the Infant classroom had several tears and exposed foam that can not be properly sanitized. The Director acknowledged the deficiency during the exit interview.
General: Based on Review of Documentation and interview, a two (2) year-old child fell while climbing on a metal ladder for a slide on the playground. Based on record review a staff member was on the playground looking at her phone when a two (2) year-old child climbed up a metal ladder for a slide and slipped and fell. Based on record review the two (2) year-old child was crying and walking around like she may have been injured immediately after the fall. The two (2) year-old child sustained a vaginal skin tear, but it could not be determined if that injury occurred from the fall on the playground. Equipment
General: Based on Review of Documentation, the facility has a phone policy in their policies and procedures manual for the staff that states: "At New Life Creations LLC the teachers' primary resposibility is to the children, therefore, no personal calls will be made or received while in a classroom. Messages will be taken and given to the teacher. Emergencies will be dealt with on a case-by-case basis. Cell phones are to be kept in the director's office and turned off or set on vibrate mode." Based on interviews a staff member had her cell phone out on 3/16/16 while she was on the playground supervising a group of children. A two (2) year-old child fell while climbing on a metal ladder for the slide. Based on interviews the staff member had her cell phone out when the accident occurred.
General: Based on Observation, a plastic slide with metal ladder rails was on the playground where the 2 year-old children play. This slide was approximately 5 feet and four and half inches tall. This slide was not developmentally appropriate for use with the two (2) year-old children. On 3/16/16 a two (2) year-old child climbed up the metal ladder steps and slipped and fell.

A flickering ceiling light was found in the Two's Room directly above a small table near the back of the room that children in care used. The light's flickering was constant and was observed by the surveyor for three minutes.
General: Based on observation, the child care center failed to maintain state regulations. During the survey of the Two's Room, the following was found: 1.) A staff member's cell phone and coffee cup were found on a small wooden cubie along the right wall, in reach of children. 2.) A Glade Plug-In was found in the front right corner of the room in an outlet that was within reach of children. It should be noted that the surveyor conducted research and found that packaging on Glade Plug-Ins contain a warning to keep the item out of reach of children and pets.

General: Based on observation, the child care center failed to maintain state regulations. Multiple areas of wild plant growth and weeds were found protruding up through the black mulch on the outside playground.
General: Based on observation, the child care center failed to maintain state regulations. During the survey of the outdoor playground, a blue climbing apparatus was found to not have an adequate fall zone. It measured three feet tall, but the fall zone on the side next to the privacy fence only measured two feet in length. General: Based on observation, this regulatory requirement was not met. During a tour of the kitchen, the surveyor observed a large, sharp knife being stored in a drawer beside the stove. Both kitchen doors were propped open during the inspection, making the area accessible to the children.

A review of staff files during the inspection found one staff person that did not have the date of employment listed in their file. General: Based on observation, this regulatory requirement was not met. The cabinet underneath the right side of the sink was in disrepair. The board on the base of the cabinet was crumbling and no longer attached to the trim. A black mat had been placed on top of the brown spots located on the base of the cabinet. A review of Division of Regulated Child Care files found that this regulatory violation was previously issued as a result of the renewal inspection conducted on 5/10/21. A review of the children's attendance record for 6/30/22, in the Two- Year-Old Classroom found that three children were signed in with arrival times documented; however, there were five children present in the classroom at the time of the visit. The staff person working in the classroom stated that she only documents arrival and departure times for children enrolled in the Child Care Assistance Program and indicated that the staff person in charge might have attendance records for children that are "private pay". The staff person in charge stated during interview that she uses a check mark system to record attendance for the "private pay" children; therefore, arrival and departure times are not documented for them.
