General: Based on observation, the following was found: Tugboat Classroom 1. The changing pad had a large tear, exposing the interior foam. This makes the pad unable to be properly sanitized. General: Based on review of documentation, the following was found: 1. Four (4) staff members (DOH: 01/08/25, 01/08/25, 01/08/25, and 01/08/25) did not complete (15) fifteen hours of annual training for the 2024-2025 calendar year. 2. Two (2) staff members (DOH: 08/19/25 and 10/14/25) did not have evidence of completing the cabinet-approved orientation training within the first three (3) months of employment. 3. Two (2) staff members (DOH: 01/08/25 and 01/08/25) did not complete the required pediatric abusive head trauma training (PAHT) within the first year of employment. A review of ECE-TRIS verified this information.

Starfish Classroom 1) An accumulation of dust on the bathroom exhaust vent. 2) An accumulation of dust on a ceiling return vent above the cots.
