General: Based on observation, the child care center failed to maintain state regulations. In Classroom Thirty-Seven (37) the water temperature in the sink utilized for handwashing measured eighty-two (82) degrees.
General: Based on observation, the child care center failed to maintain state regulations. During the survey, the following was observed: Classroom #37 1. The changing table pad had a basket with dry-erase markers for logging diaper changes. The changing table should be free of all items not used in diapering. Classroom #8 1. The diaper changing table pad had a piece of paper with daily information and a glasses case. The changing table should be free of all items not used in diapering.
General: Based on observation, the child care center failed to maintain state regulations. In Classroom Thirty-Seven (37) a ceiling tile above the maroon cabinet and a tile above the kitchen center were stained.
General: Based on review of documentation, the child care center failed to maintain state regulations. During the survey, the following was observed: 1. One (1) staff member (DOH: 07/17/22) had a TB skin test on file that had expired 08/10/24 and therefore was not current. 2. One (1) staff member (DOH: 02/09/22) had a TB skin test on file that had expired 02/24/25 and therefore was not current.
General: Based on review of documentation, the child care center failed to maintain state regulations. During the survey, the following was observed: 1. One (1) staff member (DOH: 02/09/22) only completed twelve (12) of the required fifteen (15) hours of annual training for the year dated 07/01/23 to 06/30/24. 2. One (1) staff member (DOH: 02/09/22) only completed fourteen (14) of the required fifteen (15) hours of annual training for the year dated 07/01/23 to 06/30/24. 3. One (1) staff member (DOH: 09/03/24) did not complete their cabinet-approved orientation training within the first three (3) months of employment. It should be noted the orientation training was completed 03/06/25. A review of ECE-TRIS confirmed these findings.
