Two (2) employees (DOH: 09/01/13 and 10/24/18) had renewed their pediatric abusive head trauma training after the expiry date. 2.) One (1) employee (DOH: 07/19/24) had completed their six (6) hour, cabinet approved orientation training after the first three (3) months of employment. A review of ECE-TRIS supports these findings.

General: Based on observation, this regulatory requirement was not met. The following was found: 1. Two pans in the kitchen that are used to prepare food contained numerous scratches on the surface. General: Based on observation, the surveyor found three vacuum cleaners accessible to the children in care. Two vacuum cleaners were found in the science area in the preschool classroom and the third vacuum cleaner was found next to the cots in the two-year-old classroom. General: Based on observation and interview, the programming needs of school-age children were not met. Upon arrival at approximately 8:50 a.m., the surveyor found that five school-age children ages six- to twelve-years-old were sharing a classroom with eight preschool children ages three- to four-years-old. The school-age staff person stated that the school-age children had a designated classroom, but they had not used the classroom for approximately two weeks. The surveyor noted that the school-agers were working on NTI assignments on laptops to the left side of the classroom, and the preschool children were participating in circle time to the right side of the classroom. However, the surveyor later noted that the preschool children started playing in activity centers on the left side of the classroom. At one point, a preschool-age child was observed crying loudly as he sat on a chair facing a twelve-year-old female who was sitting against the wall working on her laptop. The school-age staff person indicated that the preschool children generally do not bother the school-age children because they have on headphones. It should be noted, the surveyor found the arrangement for the school-agers not conducive to learning, but disruptive. Premises Not In Compliance

General: Based on observation, there were plastic bowls and plastic cups stored on the floor in the preschool classroom, which is not sanitary.
General: Based on observation, 1.) The staff bathroom located between the two children’s bathrooms had ten bottles of chemical cleaners on a table approximately three feet from the ground. The latch to a cabinet under the sink in this bathroom was unfastened. This cabinet housed bleach, toilet cleaner, and two containers of liquid hand-soap. There was also a plunger and two toilet brushes located beside the toilet. 2.) An opened box of plastic garbage bags was located on the floor in the one-year-old classroom. 3.) A drawer in the preschool classroom contained a “Scrub Buddy Quick Eraser.” The packaging on the sponge stated, “Keep out of reach of children." There was also an unlatched, plastic container on a desk in this classroom. A bottle of ibuprofen was located inside of the container. These items were accessible to the children in care.

General: Based on observation, there was a three-month-old child asleep in a swing with a bottle in his mouth in the Infant classroom.
General: Based on observation, the following was found: 1. There were two (2) staff purses being stored on the floor by the teacher's desk in the Twos classroom. The purses were accessible to the children in care. 2. There was a bottle of correction fluid being stored in a lower drawer in the teacher's desk in the Preschool classroom. General: Based on Interview, the staff person in charge stated that trash is being dumped over the fence onto the ground next to the playground space every day. She stated that this is done because the faciity would smell bad if the trash was left inside all day. She stated that a staff person goes outside around mid day and then again at the end of the day to pick it all up and walk it over to the dumpster which is located at the back of the lot. General: Based on Observation, the temperature in the building on 10/25/17 was sixty-five degrees. The staff person in charge stated that the gas was shut off a couple of days ago because of a new resturant being built next to their facility. She stated that the only gas they use at the facility is for heating the building so not until a couple of days ago was the heat turned on. She stated that everything else at the facility is operated by electric and the gas did not need to be turned on until recently due to the weather getting colder.
General: Based on Observation, while walking the facility two seperate issues were noted. 1. There were three to four (3-4) medium tears in the flooring showing the subfloor in the hallway which connects the front classroom to the back classroom. These were noted to be in the middle of the hallway right in front of the children's entrance to the bathroom. 2. In the two year old classroom two of the walls show excessive crayon/pen marks throughout the wall. This is located on the far left wall when walking into the room.
General: Based on Observation, it was found that during the inspection the water in the portable sink in the one year old (1) classroom was not working proprely. HSS was unable to get a steady stream of hot or cold water to flow from the sink. General: Based on Review of Documentation, out of ten (10) children's files reviewed there were two (2) children whose information did not contain the phone number of the child's physician. In addition there were two (2) children's files that did not contain information regarding a preferred hospital.
General: Based on Review of Documentation, it was found that the names of the children who participated were not documented for the monthly fire drills. General: Based on Observation, during a tour of the preschool room a note was posted on the side of a cabinet that could seen by anyone in the room. General: Based on Review of Documentation, out of ten (10) children's files reviewed there were two (2) children whose immunization certificates were no longer current. General: Based on Review of Documentation and interview, this regulatory requirement was not met. One (1) staff person (hire date: 8/17/15) did not have educational documentation on file. The staff person in charge stated that she was in process of obtaining this documentation.
General: Based on Review of Documentation and interview, this regulatory requirement was not met. An inspection of the 2002 Dodge fifteen (15) passenger van used to transport children found that the front bench seat was in dis-repair. The back and seat area on one side of the bench seat contained a large tear that had been attempted to be repaired with Duct tape. The Duct tape was not secure. The surveyor observed a large section of sticky residue from the tape and exposed foam from the tear in the seat. The staff person in charge acknowledged the finding and stated that they purchased the van and started using it to transport children in October 2015.
General: Based on Review of Documentation, and interview the following was found: 1. Twenty-four (24) children were present in the facility during the inspection; however, only fifteen (15) of them had an arrival time documented. 2. Twenty-one (21) children had an arrival time documented on the 11/30/15 attendance sheet; however, four (4) of these children did not have a departure time documented. Furthermore, three (3) children had a departure time listed on the same day (11/30/15), but did not have an arrival time listed. 3. Nineteen (19) children had an arrival time documented on 12/1/15; however, four (4) of these children did not have a departure time listed. The staff person in charge acknowledged this finding during the visit.

General: Based on Observation, in the preschool room a vent above a utility closet was not secured properly to the wall. One (1) of the screws was missing so the vent was secured only on one (1) side to the wall. The hole that the vent covered was exposed since the vent covering was not secured properly.

TYPE A CITATION Based on Observation, it was found that on 3-5-14, a staff person was working alone in the one (1) year old classroom with five (5) toddlers. The staff person stated that this was her first day and that she had been working alone in the classroom since 9:00 a.m. The staff person continued to be alone in the classroom until the end of the surveyor's visit at 11:30 a.m. During a follow-up interview on 3-7-14, the staff person reported that she was not placed with a second teacher on 3-5-14 until 12:00 p.m. She also reported that she was left alone with fourteen (14) children ages one (1) year to four (4) years old for approximately five (5) minutes later that day when the other teachers left to go to the bathroom and answer the front door. This staff person did not have a completed criminal records check or central registry check on file. On 3-6-14, it was found that a staff person hired on 1-6-14 was working alone in the one (1) year old classroom with four (4) toddlers for approximately ten (10) minutes. General: Based on Observation, it was found that on 3-5-14 a four (4) year old child was observed to be allowed to walk from the preschool classroom to the bathroom down the hall unsupervised. This infant fell asleep in the bouncy seat and slept covered with a blanket and was moved into the crib only when the Surveyor mentioned that the infant was sleeping in the bouncy seat for more than 15-20 minutes.

General: Based on observation, the surveyor found the designated school-age classroom in disarray. The surveyor observed several folded cribs, several crib mattresses, tri-fold boards, miscellaneous infant/ toddler equipment, a baby gate, toddler chairs, toddler tables, toddler sized shelving units, and ditto sheets and grocery bags scattered about the floor. It appeared that this room was being used for storage, not a school-age classroom.
General: Based on review of documentation presented for review, a staff person hired on 9/15/2020, had a criminal background check dated 11/24/2020; therefore, it was not completed prior to hire. The staff person in charge stated that this staff person was not left alone with children until the criminal background check was received.
Based on observation, the surveyor attempted to conduct a renewal inspection on 1/27/2021, but found the child-care center closed. The surveyor called the staff person in charge and found that the child-care center had been closed since 1/21/2021, due to a possible COVID exposure. General: Based on Observation, and interview, this regulatory requirement was not met. During a tour of the two (2) year old classroom the surveyor observed the following: 1. A large metal vent on the wall beside the sink/diapering area contained an excessive amount of rust. 2. An excessive amount of crayon marks was observed on the wall in the dramatic play area in the two (2) year old classroom. 3. The lime green concrete wall near the children's cubbies in the toddler classroom contained numerous areas of chipped/peeling paint.
General: Based on Observation, this regulatory requirement was not met. During a tour of the toddler and two (2) year old classroom, the surveyor observed a large bag of trash sitting on the carpet beside the children's cubbies and another bag of trash sitting outside on the deck.
General: Based on Observation and interview, this regulatory requirement was not met. The surveyor tested the water temperature at a portable sink in the toddler classroom and determined that the portable hand washing sink was not equiped with hot water. The staff person working in the toddler classroom at the time stated during interview that the sink does not have hot water; however, the staff person in charge stated that it used to have hot water.
General: Based on Review of Documentation and interview, this regulatory requirement was not met. A review of the transportation log found that daily inspections of the vehicles used to transport children had not been documented since 11/6/15. The staff person in charge stated that children are transported back and fourth to local elementary school on a daily basis.
General: Based on Review of Documentation and interview, this regulatory requirement was not met. A review of the transportation log found that the first and last names of the children transported were not consistently documented. Some of the forms listed first names of the children who were transported; however, the last names of the children were not documented. The staff person in charge identified an internal code system that the facility was using which listed the names of the children on the top of the transportation page and then the first initial of each children on each line down the page; however, a review of the forms found that first and last names were not always documented.
General: Based on Observation and interview, this regulatory requirement was not met. The refrigerator used for the storage of the bottles in the infant classroom and the refrigerator used to store food and drinks for the children in the toddler classroom, did not have an indicating thermometer. The staff person in charge stated that she was not aware that these thermometers were missing.
General: Based on Review of Documentation and interview, this regulatory requirement was not met. At the time of the survey a review of the children's files found that one (1) child (enrollment date: 9/23/15) had no immunization record on file and four (4) children (enrollment dates: 10/21/14, 10/17/14, 5/5/15 and 6/8/15) had immunization certificates that were no longer current as of 6/30/15, 9/19/15, 10/30/15 and 9/2/15. The staff person in charge stated that she was in process of obtaining this documentation.

TYPE B CITATION: Based on Interview, it was found that on 2-28-14, multiple staff members did not come to work for various reasons. The director stated that due to staff shortage, fourteen (14) children ages one (1) year to four (4) years old were combined all day in the two (2) year old classroom. There were two (2) staff supervising the children during this time. Due to the presence of one (1) year olds, the required ratio was one (1) staff person for every six (6) children. In addition to being over ratio by two (2) children, the classroom was also over maximum group size by two (2) children. It was also found that on 3-5-14, one (1) teacher was supervising fourteen (14) children ages one (1) year to four (4) years old for approximately five (5) minutes. The other teachers had left the room to answer the door and go to the bathroom. During this time the classroom was over ratio by eight (8) children and over maximum group size by two (2) children. General: Based on Observation, it was found that on 3-6-14, two (2) infants were sleeping in swings. The director stated that the children take their naps in the swings at the parents' request. The facility did not have documentation on file from a physician stating that it was necessary for the children to sleep in this fashion.
TYPE A CITATION: Based on Observation, t was found that on 3-5-14, a staff person was working alone in the one (1) year old classroom with five (5) toddlers. The staff person stated that this was her first day and that she had been working alone in the classroom since 9:00 a.m. The staff person continued to be alone in the classroom until the end of the surveyor's visit at 11:30 a.m. During a follow-up interview on 3-7-14, the staff person reported that she was not placed with a second teacher on 3-5-14 until 12:00 p.m. She also reported that she was left alone with fourteen (14) children ages one (1) year to four (4) years old for approximately five (5) minutes later that day when the other teachers left to go to the bathroom and answer the front door. This staff person did not have a completed criminal records check or central registry check on file. On 3-6-14, it was found that a staff person hired on 1-6-14 was working alone in the one (1) year old classroom with four (4) toddlers for approximately ten (10) minutes. General: Based on Observation and interview, the facility failed to meet this regulatory requirement. During an inspection of the kitchen with the staff person in charge, the surveyor only found whole milk and 2% available to serve to the children. The staff person in charge stated that she was not aware of this regulatory requirement and stated 2% milk was served to ages twenty-five (25) months and older during breakfast on the day of the survey.

During the survey the staff person in charge presented the food and drink items that were being prepared in the kitchen that were going to be served to the children at lunch. During an inspection of the kitchen with the staff person in charge, the surveyor observed that cups of juice had been arranged on trays to be served with the meal instead of milk. The staff person in charge stated that she was not aware of this regulatory requirement.
