During a follow-up inspection, the surveyor entered the Infant classroom at approximately 12:55 p.m. and observed four (4) children present in the Infant classroom ranging in age from six (6) months to fifteen (15) months. It is important to note, the maximum capacity of the Infant classroom is three (3) children.

The facility was measured by a surveyor on 6/21/2022 and the Infant Room's capacity was three (3) children.

General: Based on observation, there was an infant in a crib holding and drinking a bottle. The staff person was sitting by the wall between the other two infants in bouncy seats; therefore, not holding the child drinking the bottle.
General: Based on observation, upon arrival to the facility on 06/14/2022, there were 14 children in the School-Age classroom ranging in age from Two-Years-Old to Fifteen-Years- Old. The surveyor noted that the classroom did not appear big enough to house that many children. On 06/21/2022, the surveyor visited the facility to measure each classroom used by children. The School-Age classroom was measured and it was determined that only eight children would be approved for that room. The School-Age room on 06/14/2022 had 14 children so the classroom was over capacity by six children.
General: Based on review of documentation, on 06/14/2022, the surveyor found one staff person working in the School-Age classroom who had previously lived in Texas. The out of state background check had been submitted for the staff but the staff person in charge stated that the facility was still waiting on the results. The staff person was working alone in the classroom for approximately five minutes. General: Based on observation, upon arrival at 8:40am, one staff person was found caring for 14 children in the School-Age classroom ranging in age from Two-Years-Old to Fifteen- Years-Old. The staff person was alone for approximately five minutes until a second staff person entered the classroom. Per interview with the staff person in charge, another staff person was scheduled to work at 9:00am and take some of the children to another classroom. General: Based on Review of Documentation, four (4) staff did not complete the fifteen (15) hours of cabinet-approved early care and education training during their completed year of employment. These four (4) staff were all hired on 2/25/16. One (1) staff hired on 2/25/16 did not complete the one and one-half (1.5) hours of pediatric head trauma training. General: Based on Observation, the kitchen contained a cabinet with approximately seven (7) knives. The cabinet contained a safety latch that was broken. In addition, under the sink in the kitchen was a half-gallon of Great Value Bleach. This cabinet was unlocked. The door to the kitchen was open when the surveyor arrived to the facility. The school-age room contained a gallon of bleach on a shelf within reach of a school-age child. The infant room contained push pins located in the ceiling. The hall bathroom, used by the preschool and school-age children, contained a can of Great Value Hawaiian Spray Refill under the sink and it was labeled "Keep out or reach of a child". The hall bathroom closet was unlocked and it contained three (3) gallons of paint, odorless lighter fluid, Tilex Mold and Mildew Cleaner, glass cleaner, and Liquid Plummer.
General: Based on Observation, the preschool room did not have a daily schedule posted. Animals In Compliance
General: Based on Observation, it was found that there was a torn diaper changing pad in the diapering area. The staff in charge stated that she would get rid of the diaper changing pad.
General: Based on Observation, it was found that there was a section of flooring missing in the Before and After school classroom. The staff in charge stated that it was due to the owner tearing out a wall in that classroom. It was also found that there were several areas of chipped paint on the walls in the Before and After school classroom and in the three (3) to four (4) year old classroom. It was found that the circular alphabet rug in the three (3) to four (4) year old room was covered with several stains. It was found that the vent in the two (2) year old classroom was covered in dust. It was found that the tile around the window in the bathroom used for toileting was missing several pieces of tile. It was also found that peeling paint on the bathroom door.
General: Based on Observation, it was found that a wall separating the older school age room and the office space and a wall separating the four (4) year old room and the younger school age room were removed from the facility. There is not evidence that the staff in charge contacted the Kentucky Department of Housing,Building and Construction, the State Fire Marshal's Office or the cabinet concerning the renovation.
General: Based on Review of Documentation, it was found that the file for an employee hired on 2-25-16 contained evidence of a central registry check request being submitted. The results of this check had not yet been received. During the survey it was found that this employee was working alone with three (3) four-year old children. This employee worked alone with the children for the duration of the survey. 305 - Background checks/discharged Discussed 922 KAR 2:090. Section 6. License Issuance. This play wooden freezer and refrigerator were not constructed with the handles on the outside of the doors. The handles were located inside both doors, making it extremely difficult for a child to open both doors. In addition, the wooden oven did not have a handle on the outside. The surveyor was unable to open the oven since there was not a handle or an area to grasp; therefore, it was undetermined if the oven had a handle.

General: Based on observation, the following was found: 1) There was a large squirt bottle found underneath an unlocked bathroom sink cabinet next to the infant classroom with a purple substance found inside the bottle. There was no label on the bottle. 2) A disenfectant cleaner and a can of Glade was found under an unlocked sink cabinet in the bathroom next to the preschool classroom. 3) There were several cans of paint found in an unlocked sink cabinet in the kitchen. General: Based on Review of Documentation, four (4) children's records were missing the physician's name and phone number. One (1) other child's file was missing a phone number to contact the child's physician. Two (2) children's files were missing a preferred hospital. General: Based on Interview, it was found that there was no staff in charge to carry out the director's duties. The staff did not have access to staff files, children's files or other relicensure documentation. When asked who was in charge, it was translated to a parent dropping a child off that no one present at the facility was in charge.
General: Based on Observation, it was found that there were several sharp knives in a drawer in the kitchen. A gallon of Clorox cleaner and a bottle of Fabuloso cleaner were observed to be in an unlocked cabinet under the sink in the kitchen. The doors to the kitchen were opened.
General: Based on Observation, it was found that there were six (6) torn mats with the foam exposed in the three (3) to four (4) year old classroom. The staff in charge stated that these mats were used for napping.
General: Based on Observation, it was found that the gate at the door in the two (2) year old classroom was broken. It was found that the faucet in the bathroom used by the children for toileting was coming loose from the sink. It was also found that the metal door closer at the top of the kitchen door was broken. The staff in charge stated that she would inform the owner of what needed to be fixed.
General: Based on Observation, it was found that the posted diagram of the facility was not accurate based on the locations of the rooms used for childcare. The diagram indicated that there were two (2) dining areas and four (4) classroom areas. The director stated that the diagram was based on the previous owner's usage of space. There are now six (6) classrooms being utilized for childcare.
General: Based on Observation, it was found that the bathtub located in the bathroom used by the older children was discolored with an accumulation of grime. A black substance was observed to be growing on the windowsill and the edge of the window located in the bathtub. The director stated that she hopes to remove the bathtub and make better use of the space.
