General: Based on review of documentation, the personnel record for a staff member (DOH: 03/30/26) contained the most recent TB skin test dated 02/09/24, which was no longer current.
General: Based on observation and interview, in the EPS One (1) Classroom, two (2) ceiling tiles, located in the middle of the classroom and near the window, had brown stain marks on the outer surface and were not in good repair. A staff member indicated a water leak occurred when it snowed and the tiles were replaced but continued to develop brown stains.
General: Based on review of documentation, the following was found: 1) A child (DOE: 01/19/26) had an immunization record on file that expired on 04/01/26. 2) A child (DOE: 01/19/26) had an immunization record on file that expired on 04/06/26. 3) A child (DOE: 03/17/26) had a current My Chart immunization print out on file that did not contain a physician's signature.

General: Based on review of documentation, the following was found: 1) A child (DOE: 01/19/26) did not have the childcare center's documents that would contain her medical history, her pediatrician with contact information and her preferred hospital with contact information, in the file. The child had the rest of her enrollment packet in her file but not those documents. 2) A child (DOE: 02/17/26) had a signed enrollment packet on file with the medical history, pediatrician information and preferred hospital information were left blank. 3) Five (5) of the ten (10) children files reviewed did not have contact information for the listed preferred hospital.
General: Based on interview and review of documentation, in the Toddler Two (2) Classroom, the lesson plan was inside a clipboard and dated 03/30/26--04/03/26. The staff member in the classroom stated that she was out sick the prior week and forgot to change it when she returned to work.
General: Based on interviews, it was determined that on 04/02/26, a three-year-old child was unsupervised after going to the bathroom in the Pre K 2 Classroom, where he closed the door, exited, and entered an unoccupied classroom on the opposite side.
