At the time of the Investigation conducted on 05/15/2025, it was determined that Staff Person #1 did not immediately make a report of suspected child abuse to ChildLine when informed by the parent of Child #1 of allegations of child abuse occurring at the facility.
At the time of the Investigation conducted on 05/15/2025, it was determined that the provider did not notify the appropriate regional office within 24 hours after receiving notice that Child #1 received emergency room treatment.

At time of inspection, room #1 was out of compliance with room capacity 9 children (5 Infants and 4 YT) in a room that measured for 5 children.

At time of inspection, during diapering of children in the infant room (#1) the staff person needed to be reminded to sanitize the changing table after use.
