At the time of the Unannounced Visit on 06/16/2026, the sink located in the Preschool Classroom was clogged.
At the time of the Unannounced Visit on 06/16/2026, in the Older Toddler Classroom, Staff Person #2 was observed changing a child on the changing table, there were 4 additional children present in the childcare space, Staff Person #2 stated they had 6 children. There were 5 children observed in the child care space and 1 child in the bathroom. Child #1 was observed in the bathroom unsupervised with their hand in the toilet. Also, in the Infant Room, Staff Person #3 was in the closet with the door closed, leaving 4 children unsupervised. THIS IS CONTINUED NONCOMPLIANCE.
At the time of the Unannounced Visit on 06/16/2026, Staff Person #3 had 4 children sleeping in the Infant Room, in cribs that were not at least 2 feet apart on at least 3 sides while the rest equipment was in use.

At the time of the Unannounced Visit on 06/16/2026, in the Preschool Classroom, Staff Person #1 was unable to name the children in their assigned supervisory group, leaving 3 children unaccounted for. THIS IS CONTINIUED NONCOMPLIANCE.
This unannounced monitoring inspection recorded no violations or advisories.
On 05/05/2026, while under the supervision of Staff Person #1, Child #1 was able to climb the outside of the indoor play structure, which is approximately 14 feet tall, falling from the structure when Staff Person #2 called Child #1's name, hitting their face on the floor, resulting in nasal bleeding, a black eye and swelling and tearing of upper lip.
At the time of the Unannounced Monitoring Visit conducted on 05/14/2026, Child #1 was placed in a crib on their back, on top of a boppy pillow with a loose blanket over the child being used to prop the bottle up to their mouth.
During the complaint investigation conducted on 03/30/2026, when asked to identify their supervisory groups, Staff Person #2 and Staff Person #3 did not have clear supervisory groups. Staff Person #2 identified 3 children in their supervisory group, but one child they named was not present at the time of the investigation. Staff Person #3 stated they could not name their group because they were in the bathroom when Staff Person #2 identified their group and they did not know which children Staff Person #2 already identified.
During complaint investigation on 03/30/2026, it was determined on 03/27/2026, child #1 eloped from Kid Zone play space as staff person #1 was leaving room. Child #1 went through lobby area, upstairs to 2nd floor where child #1 was able to access elevator, taking elevator to lower basement level, exiting through exterior door on lower basement level, running up the ramp, through the parking lot and down the sidewalk into traffic on busy main road, where a passerby and child care staff were able to retrieve child #1.
At the time of the Unannounced Visit on 03/30/2026, the file for Staff Person #1, hired 09/16/2025, did not contain verification of the completion of Pediatric First Aid and Cardiopulmonary Resuscitation Training.

During unannounced inspection 08/08/2024, stained ceiling tiles were observed in the following areas: on the first floor next to the air conditioning vent, near the fire alarm sensor and near the rest room; on the second floor in the child care space near the air conditioning vent, by the window on the left side of the classroom and in the corner near the bank of windows facing Old York Rd.
During complaint investigation on 05/22/2024, it was determined that on 05/17/2024 during nap time in classroom where 9 children were napping and two children were not, child #1 was able to open a child safety gate, take elevator down from the 2nd floor, leave main door of facility and go 0.7 miles away to Valley Rd where he was found by a passer by. On 05/22/2024, child in first floor classroom was observed eloping from classroom and cert rep alerted staff so staff could retrieve child from hallway outside of classroom.
During complaint investigation on 05/22/2024, it was determined that on 05/17/2024 during nap time in classroom where 9 children were napping and two children were not, child #1 was able to open a child safety gate, take elevator down from the 2nd floor, leave main door of facility and go 0.7 miles away to Valley Rd where he was found by a passer by.
