The center does not have toys and other materials to encourage children's creativity, social interaction, and balance of individual and group play. Immediate- Director states she added new toys, shelf and tables. Preventive- Director states she will remind to sanitize toys by area. To prevent areas to lack toys. Monitoring- Director states she will come up with a schedule of sanitation of toys.
The center groups children by age. The required staff/child ratio for the Age in group: toddlers - 1:6 or fraction of group thereof. The observed staff/child ratio was 7 children for 1 staff. Immediate-Director stated will tell her staff to ask for help as soon as needed. Preventive- Director reviewed and updated staff scheduled to ensure compliance. Educators did not complete the following training within 3-months: first aid and cardiopulmonary resuscitation (CPR) certification (11 out of 27 Staff) Immediate: On 1/15/26 Director Signed up all staff for CPR Training. They will attend on the 4th of February Preventative: On 1/15/25 Director will schedule earlier classes for new staff and will send to sister schools for trainings. From the review of staff records, it was determined that 19 out of 19 staff records does/do not include a professional development plan based on seven areas of competency. The center exceeded the allowable number of children stated on the license by either total or age as evidenced by the capacity in the two year old classroom was exceeded by four children.. Corrected on site. Director and educators will ensure capacity is maintained at all times.
Of the 11 children's records reviewed, 3 is/are missing a signed parent or guardian acknowledgement that the parent handbook had been read and understood. See the Children's Records 8.9.4.22 form for the child(ren) who have this missing. Director will request acknowledgement of handbook from the parent.

During the inspection, it was observed that the educator in the Preschool classroom was not calm when a stressful situation occurred when an educator became upset when a child had a potty accident. Director will remind staff to remain calm and respond to children appropriately.
In the 2 yr. old classroom(s) the staffing/child ratio was exceeded when there was one educator alone with 21 children for 20 minutes. Director will ensure that the educators maintain ratios at all times. Review of the regulation will be reviewed by the educators in the two-year-old classroom.

There were 35 children in the classroom for 15 minutes in the morning.. Director stated that there was no school on 11/8/2023 and it took a little longer to separate the age groups. Director will monitor the classroom capacities and review policy with staff.

The center exceeded the allowable number of children stated on the license by either total or age as evidenced by 50 children in the Pre-k classroom which has a capacity of 28. The director will utilize other classrooms to stay within posted classroom capacity.
The facility did not plan daily meals and snacks to include monitoring of portions. Director will ensure the educators will monitor serving sizes.
The center failed to inform parents or guardians before a pet was allowed in the center.
