8.9.4.21 · C INCIDENT REPORTING REQUIREMENTS
The center failed to notify the parent or guardian in the event of a head injury. Immediate: Director will continue to do incident reports and notify the licensing authority of the incident, and provide the appropriate care needed. Preventative/Monitoring: Director will do a "back to the basics: regulations training with staff during the summer for staff orientation week. After making a report to the appropriate authorities, the licensee shall notify the licensing authority of the incident giving rise to its report as soon as possible but no later than 24 hours after the incident occurred. A report should first be made by telephone and followed with written notification. The licensee shall report any incident that has threatened or could threaten the health and safety of children and staff members, such as, but not limited to: Finding The center failed to make a report to the licensing authority within 24 hours after the incident occurred regarding the abuse or neglect of a child. By evidence of report coming from SCI (CYFD) and not the licensed facility. 8.9.4.23 · B STAFF QUALIFICATIONS AND TRAINING
From the review of staff records, it was determined that 1 out of 7 staff does/do not have documentation of the 45-hour entry level course or an approved equivalent prior to or within six months of employment. 10/1/2024 The director talked to educator today and she will bring training certificates tomorrow. The director will send the checklist with the educator to make sure she has all certificates for training file. Director will continue reviewing staff files twice a month to be in compliance.
