Child care staff shall not allow children younger than 3 years of age to have access to toys, toy parts, and other materials that pose a suffocation or choking risk or are small enough to be swallowed, including, but not limited to, coins, balloons, exposed foam padding, or empty plastic bags.

The licensing coordinator observed a 21-month-old child in the preschool classroom engaging with small toys, which presented a potential choking hazard. Corrective action: Toys that were not age-appropriate have been removed from the classroom and replaced with materials that are appropriate for the children in that room.

The licensing coordinators' review of staff attendance records indicated that, in the preschool room, Staff A was not signed in for the day. Additionally, Staff B did not sign out upon leaving the preschool classroom and did not sign in upon arriving at the toddler classroom, and Staff C did not sign in at the start of their shift in the infant room or upon returning from their break. Corrective action: Staff will now be strictly using Brightwheel to track both staff and child attendance. Previously, we were using both paper and the app, and it became too much to manage. Moving forward, everything will be recorded directly in Brightwheel to keep things consistent and organized.

Child care staff shall supervise every child in care at all times.

Program staff self-reported that Child A in the Toddler classroom was left unattended outside on the program's playground. The length of time could not be definitively confirmed, but did not exceed 14 minutes. Corrective action: The corrective action plan for the event that happened on November 18th is as follows. Staff members are now to be doing a name-to-face check on their clipboard as well as counting each individual child. There is to be a teacher in the front of the line and a teacher in the back of the line. They are to be counted before they go out. They are to be counted once the children are outside. Same thing follows for when they come inside. All staff members in the room are to count and agree on the ratio number before moving forward. After all of the children are inside, one of the teachers is to go outside and do a yard sweep of the play yard. Another plan that we put in place is there are no children allowed past a certain mark on the playground to ensure that there is 100% supervision at all times and no blind spots.
